Objective To evaluate the damage on rabbits’ kidney at different infusion time with constantpressure of renal pelvis.Methods We selected64New Zealand white rabbits and established animal models accordingto renal pelvis infusion time, which was30min,60min, and90min respectively. A sham-operationgroup served as controls. There were16rabbits in each group. Normal saline was used asperfusate and the infusion pressure was maintained at100cmH2O. Then we colleted the urine fromeach rabbit from the infusion side the first day and the fifth day after operation and detected the24h urinary protein respectively, and then killed all the rabbits to take the kidney specimens of theinfusion side, and conducted HE staining and transmission electron microscope examination.Results the24h urinary protein one day after infusion:the content of the sham-operationgroup,the30min infusion group, the60min infusion group and the90min infusion group are(0.170±0.006),(0.682±0.031),(0.772±0.013),(0.851±0.037)g/24h,all comparisons between groupsexist statistical difference(P<0.05); the24h urinary protein five days after infusion: the content ofthe sham-operation group, the30min infusion group, the60min infusion group and the90mininfusion group:(0.164±0.013),(0.180±0.007),(0.232±0.022),all comparisons between groupsexist statistical difference(P<0.05); the content of the24h urinary protein of infusion groupsbetween one day after infusion and five days after infusion exist statistical difference(P<0.05); thecontent of the24h urinary protein of the sham-operation group between one day after infusionand five days after infusion not exist statistical difference(P>0.05). HE staining showed that: onthe first day after operation, the sham-operation group: the structure of the glomerular is normal,endothelial cells and mesangial cell of glomerular are not edema, the structure of renal tubular isneat, the epithelial cells are cubic and not edema; the30min infusion group: the volume of renaltubular epithelial cells increased obviously and mild edema, its cytoplasm is granula; the60mininfusion group: the glomerular mild atrophy, the walls of renal tubulars are thin, the proteinurinary casts found in some renal tubule; the90min infusion group: epithelial cells of renaltubular obviously edema, much the protein urinary casts are found in tube cavitys. on the five dayafter operation, the sham-operation group: The capillaries in renal is expansion and congestion,the structure of the glomerular is normal, endothelial cells and mesangial cell of glomerular arenot edema, the structure of renal tubular is neat, the epithelial cells are cubic and not edema; the30min infusion group: the volume of renal tubular epithelial cells increased little and mild edema;the60min infusion group: the volume of renal tubular epithelial cells increased obviously andmild edema; the90min infusion group: the renal tubule severe edema, epithelial cellsbecomenecrosis andcell casts found in the renal tubule. The transmission electron microscope showedthat: on the first day after operation, the sham-operation group: renal tubular microvilli arrangedneatly, epithelial cells no edema, the endoplasmic reticulum, the lysosome and the nucleus no obvious abnormal; the30min infusion group: epithelial cells of renal tubular edema obviously andorganelles are normal; the60min infusion group: the lysosome increased obviously, endoplasmicreticulum expansion obviously, microvilli arranged not neat; the90min infusion group: red bloodcells are found in renal tubular, microvilli arranged not neat, the lysosome increased obviously,heterochromatin of the nucleus increased obviously. on the five day after operation, thesham-operation group: renal tubular microvilli arranged neatly, epithelial cells no edema, theendoplasmic reticulum, the lysosome and the nucleus no obvious abnormal; the30min infusiongroup: the endoplasmic reticulum edema mildly, microvilli arranged neatly, organelles are normal;the60min infusion group: microvilli arranged not neat, some abnormal material found in renaltubular and intercellular become broad; the90min infusion group: epithelial cells of renal tubularedema obviously, the lysosome increased obviously, the endoplasmic reticulum cystic expandobviously.Conclusions When the perfusion pressure of renal pelvis is100cmH20and the infusion time ofrenal pelvis is between30minutes and90minutes, the structure and function of the kidney willdamage in different degree and the longer the infusion time was, the more serious the damage was,but the damage was reversible in different degree in this experimental conditions.So within thescope of the perfusion pressure and infusion time, the damage is reversible.
