Objective1. To evaluate the problems and factors in the operation of the self-eyedrops of glaucoma patients after the operation.2. Combined bacterial culture to assess the pollution of patients using eye drops3. Cause clinicians’attention about self-management of after glaucoma surgery patients.Methods1.209Cataract and glaucoma surgery patients were randomly (107glaucoma,102cataract), After glaucoma surgery patients as the observation group, Cataract surgery patients as a control group.Subjects were video recorded self-instilling a drop onto the postoperative eye.All patients expressed complete the the eyedrops process by their own at home.2.1nform patients who participate video record review eye condition In the out-patient after7days of and carry eyedrops.Take the first two drops of patient’s eye drops,respectively for bacterial and fungi culture.Gram stain and observe culture results, recorded positive rate and type of bacteria.Results1. Multivariate regression analysis showed that the number of drops is related to patient’s age(B=0.013,P=0.003) and illness(B==0.254,P=0.003);the times of eye drops bottles contact with skin are related to the patient’s age(B=0.031,P=0.000) and illness(B=0.268,P=0.003).Correlation test shows:the number of drops and the times of eye drops bottles contact with skin is positively correlated with the patient’s age (Spearman Coefficient0.371,0.532, p<0.05).Drops of group G (181) are more than group C (145)..difference was statistically significant (Z=-3.120, p<0.05);the times of eye drops bottles contact with skin of group G (194) are more than group C (99) difference was statistically significant (Z=0.2717, p<0.05)2. In the present study, only one patients discard the first drop of liquid eyedropper;Two patients appeared eye pain, tearing after self-instilling the eyedrops and were diagnosed with corneal abrasion.3. Bacterial culture,collected eye drops specimens at a total of191cases(glaucome97.cataract94).Bacterial only growth on the petri dish with pranoprofen,The other two eyedrops in the culture dish were no microbial growth.All the samples showed no fungal growth.4. The first eye drops bacterial culture results show that:the positive rate of glaucoma patients were significantly higher than cataract patients (χ2=9.03,P<0.01);he second eye drops bacterial culture results shows that:the positive rate of glaucoma patients were significantly higher than cataract patients (χ2=5.26,P<0.05).The overall culture results show:bacterial culture positive rate of the first eye drops is higher than the second drops (χ2=17.16,P<0.01)5. Staphylococcus epidermidis are the most in the results of bacterial culture(48.4%),Followed by Gram-negative bacteria(41.1%);Staphylococcus aureus were detected two cases in the first eye drops,but none in the second drops.Conclusions1. The video observation shows that there are many obvious mistakes of glaucoma and cataract after operation patients in self-instilling eyedrops. Performances of glaucoma after operation patients were worse than cataract after operation patients.2. Compared with ordinary cataract after operation patients,glaucoma after operation patients with a high risk of eye drops contaminated.3. Discard the first eye drops may reduce the risk of associated infections effectively.Therefore, clinicians should attach great importance to the related education and the instruction of the after operation patients,Inform patients the importance of correct and effective use of eye drops and how to use eye drops correctly.
