Correlation Study Between Serum YKL-40and Acute Cerebral Infarction

Objective:To study the correlation among serum YKL-40, different classification of acute cerebral infarction, stability of carotid plaques and low-density lipoprotein level of correlation.Methods:87patients with acute cerebral infarction from the second department of neurology of Second Hospital of Shanxi Medical University were chosen as case group and patients have undergone CT classification, TOAST classification and OCSP classification;65cases with no apparent performance of cerebrovascular disease matching the case group were chosen as the control group; to understand the level of narrowing lumen and stability of carotid plaque through the color Doppler ultrasound examination of carotid artery; all selected people were tested by low-density lipoprotein and high-sensitivity C reactive protein (hs-CRP), and their serum YKL-40levels were detected by ELISA; statistical methods were used to analyze the correlation among the serum YKL-40, different classification of cerebral infarction and stability of carotid plaques, at the same time a comparison with YKL-40, hs-CRP and cerebral infarction was made.Results:1. Serum YKL-40level of case group was significantly higher than the control group (P<0.05).2. YKL-40level of unstable plaque group was significantly higher than stable plaque group(P<0.01)and none plaque group(P<0.01), YKL-40level of stable plaque group was significantly higher than none plaque group (P<0.01).3. YKL-40levels were diverse in different cerebral infarction patients (P<0.05), in the imaging CT classification, YKL-40levels of each group were higher than the control group, YKL-40level of large infarct group, medium infarct group, multiple infarct group, small infarct group and lacunar infarct decreased gradually, which were statistically significant (P<0.05); in the TOAST classification, YKL-40levels of LAA group, CE group were much higher than the control group (P<0.05); however, YKL-40level difference of SAA group, SOE group and the control group has no statistically significant (P>0.05); in the OCSP classification, YKL-40levels of each group were higher than the control group, which were statistically significant (P<0.01); and PACI group has the highest level, however.4. The serum YKL-40level of case group has no significant correlation with hs-CRP (r=0.063, P>0.05).5. Low-density lipoprotein in the case group has a positive correlation with the rising of YKL-40level.(r=0.229, P>0.05).Conclusion:Serum YKL-40level reflected the stability of carotid plaques and was related to the rise of low-density lipoprotein. Serum YKL-40might point out infarct size, the severity of patient’s condition, the etiology and prognosis of cerebral infarction patients, which was independent of hs-CRP and has a guiding role in the comprehensive treatment of patients with cerebral infarction.
