PurposeBy observing the curative effect of Yiqihuayu Decoction in preventing the postoperative recurrence of endometriosis (EMT), this thesis was to research pathogenesis of postoperative recurrence of EMT, and to discuss mechanism, feasibility and effectiveness of Yiqihuayu Decoction in preventing postoperative recurrence of EMT.MethodsThe study was performed by choosing62Endometriosis patients from December2010to December2012who had operation in Hubei Province Chinese Medicine Hospital, Wuhan Integrated TCM and Western Medicine Hospital, or other hospitals. The selected patients were randomly divided into two groups as Chinese medicine treatment group(CMTG,31cases) and Western medicine control group (WMCG,31cases). CMTG were treated with Yiqihuayu Decoction while WMCG were treated with gestrinone in preventing postoperative recurrence of EMT. The treatment was discontinued after there months. All patients were required to visit outpatient on the time of withdrawal and after withdrawal for six months. The following indexes have been observed for comparison, including side effects of the medication and amenorrhea situation when treating, and the relapse rate, dysmenorrhea, pregnancy rate, CA125after treating. B-mode ultrasonography inspection and gynecologial examination were also conducted as well. When the patient was pregnant or recurrence of EMT, observation would be terminated. All data were processed by SPSS16.0. Quantitative data with x±s, t test; count data by%test; unknown distribution data with rank sum test.Results1Relapse rate:the recurrence rate of CMTG or WMCG by the end of one course of treatment was0%. The recurrence rates were6.45%and9.68%, respectively at six month after withdrawal. There was no significant difference (P>0.05) between two groups regarding the recurrence rate. It indicated that the treatment of both groups was effective and equivalent in preventing the recurrence of EMT.2CA125value variation:the CMTG and the WMCG in reducing CA125values had equivalent efficacy (P>0.05). After withdrawal CA125values had returned to normal for both groups, and it always decreased in withdrawal for six months. It concluded that the two groups shared similar efficacy in reducing CA125values, and curative effect can last6months.3The recurrence of dysmenorrhea:in avoiding dysmenorrhea recurrence and reducing the symptom score of dysmenorrhea, the two groups were of statistically significant difference (P<0.05) after withdrawal for six months. The recurrence rate and symptom score of CMTG were significantly lower than WMCG.4pregnancy rate:at withdrawal, the pregnancy number of CMTG was3, while WMCG was0. At6months after withdrawal, the pregnancy number of CMTG was7(53.85%). Respectively, the pregnancy number of WMCG was2(14.29%). The two groups were of statistically significant difference (P<0.05). In improving the patient’s pregnancy rate, the efficacy of CMTG was better than WMTG.5Side effect:the occurrence of medicine caused side effects in the CMTG was2, while the number with side effect was15in the WMCG. The two groups had a significant difference (P<0.01). The treatment of CMTG has advantages on less side effect and higher safety.6Menstrual situation:during treatment, the CMTG did not cause amenorrhea or abnormal vaginal bleeding, while those were observed among all patients in WMTG. There was significant difference (P <0.01), illustrating that the CMTG treatment had an advantage without affecting the patient’s normal menstrual cycle.ConclusionThrough the observation of the curative effect of Yiqihuayu Decoction for the prevention of postoperative recurrence of EMT, we found that Yiqihuayu Decoction and gestrinone have the comparable effects in reducing relapse rate and CA125values. In the aspects of increasing the pregnancy rate, reducing the dysmenorrheal recurrence rate and dysmenorrheal score, Yiqihuayu Decoction was significantly better than gestrinone. Yiqihuayu Decoction had little side effect after treatment. It did not cause amenorrhea and affect the postoperative gold period of pregnancy (in1year). It could regulate menstruation, help pregnant and promote postoperative recovery. It had been clinically proved effective, and had a good application prospect.
