Objective To obtain the characteristics of the glucose fluctuation in pregnantwomen with different state blood sugar condition by continuous glucose monitoring system(CGMS).To analyze the reliability of the CGMS which is used in the pregnant wome.Methods From January in2011to October in2012.,according to ADA2010diagnosticcriteria,41individuals were divided into2groups: normal glucose tolerance ofgestational(NGRG, n=13),newly diagnosed gestational diabetes mellitus(GDM, n=28).Pregnancy women who with diabetes, hypertension, other endocrine and metabolic diseases,smoking and alcohol need to be excluded. Take the survey and the basic laboratory tests of thepregnancy women,continuous glucose monitoring system (CGMS) was used for72hours tomonitor the blood glucose level,to analyze the indicators of the clinical of the CGMSsimultaneously.Results1. Comparison of the regular indicators:There was no significant differencebetween the two groups in BMI,blood pressure,blood serum creatinine,blood ureanitrogen.The GDM group in age,gestational weeks,weight gain during pregnaney,bloodglobulin, triglycerides,cholesterol all were higher than that in the NGRG group;The GDMgroup in blood albumin were lower than that in the NGRG group(p<0.05).2.The deviationbetween the CGMS and the SMBG of the pregnant wome:.the state of the blood glucose levelbetween the CGMS and the SMBG:the BG lower than3.9mmol/L was10.2%,higher than8.3mmol/L was7.4%,and there is no deviation of the normoglycemia range.3.Thecharacteristics of glycemic excursion in the two groups:There was no significant differencebetween the two groups in the mean Blood Glucose(MBG), large amplitude of glycemic excursions(LAGE),Mean of Daily Difference(sMODD)And the standard deviation of bloodglucose(SDBG)、the mean amplitude glycemic excur-sions(MAGE)of the GDM group bothsignificantly higher than that of the NGRG group.The fasting blood-glucose、1hPG、2hPG、3hPG levels in the GDM group were higher than the NGRG group.The postprandial bloodglucose of the GDM group which reach the peak and botton both later than the NGRGgroup.But there was no significant in the bottom of the blood glucose after lunch.4.The meannocturnal blood gloucose of the GDM group was hiher than the other group,and the time ofthe incidence of nocturnal unrecognized hypoglyce is longer than the NGRG group.Conclusion1.The weight increased of the GDM was higher compared with the normalpregnant,which may be related to its etilogy.In pregnant women with higherfibralbumin,indicating the increasingly immune.CGMS could offer the spectrum of thefluctuations in blood glucose more perfectly,it could find the incidence of nocturnalunrecognized hypoglyce and the hyperglycemia.And the measured value by the CGMS wasWell-matched with the peripheral blood glucose.GDM must pay attention to the1hPG,because1-1.5h later the postprandial blood glucose of the GDM group could reach the peak of theblood glucose.Meanwhile,hypoglycemia has lasted for a long time in GDM,the GDM shouldpay more attention to the incidence of nocturnal unrecognized hypoglyce.2. The within-day,day-to-day, and postprandial glucose excursions were evaluated retrospectively by CGMSfor72hours. The incidence of nocturnal unrecognized hypoglycemic events detect in time,andit may be helpful to choose the treatment programs which more rational andindividualized.Then the blood glucose of the GDM could reach standard more faster and moresecure,reduce the complications of the perinatal period,and get a good long-term outcome.
