ObjectiveTo analyse the charaeterisitics of daily blood glucose profiles of insulinoma using continuous glucose monitoring (CGM) and to investigate the value of CGM in the diagnosis of insulinoma.MethodsThe blood glucose of13patients with pathologically-confirmed insulinoma (insulinoma group) were detected by CGM for3consecutive days. The mean blood glucose (MBG), mean amplitude of glycemic excursions (MAGE), frequency distribution of glucose levels and the hypoglycemic episodes of CGMS were measured, and the results were compared with those of normal glucose regulation (n=10, control group) and patients with newly-diagnosed type2diabetes with reactive hypoglycemia (n=11, diabetes group).ResultsThe MAGE of insulinoma group was significantly higher than that of control group (P<0.05), while there was no significant difference in MBG between the two groups (P>0.05). The MAGE and MBG of diabetes group were higher than those of insulinoma group and control group (P<0.05). As a whole,81hypoglycemic episodes lasting4.2hours per day occurred in the13patients with insulinoma, of which60.4%(49episodes) took place during the night hours. While16hypoglycemic episodes lasting0.13hours per day were observed in the11patients with diabetes, of which9episodes (56.2%) were detected during the night hours.99.4%of the findings of the blood glucose of control group fell in the range between3.9and7.8mmol/L. The frequencies of glucose below2.8mmol/L and between2.8and3.9mmol/L of insulinoma group were siguificangly higher than those of diabetes group (P<0.05), while those above11.1mmol/L and between7.8and11.1mmol/L were lower than those of diabetes group (P<0.05).ConclusionsThe time distribution, type and cause of hypoglycemia could be well analysed by CGM, and parameters for blood glucose such as MAGE revealed by CGM could help in the diagnosis of insulinoma.
