Objective To master the status and the influencing factors of mild cognitive impairmentabout the people aged55years and above and make a clear of the correlation between thepolymorphism of apolipoprotein gene E and mild cognitive impairment.Method The survey includes a total of2168people in the city and rural area.The PPSsampling design is used in the rural area, in2230selected households,a face-to-face way isadopted to make a research about all the people aged55and above complying with the objectof study criteria,it relates to the number of939people (a percentage of43.3).A typicalsampling method is used for the city residents,the survey selectes five city communities with ahigher proportion of elderly population in the city Yinchuan and Wuzhong of Ningxia,itaccounts a total of1229people (a percentage of56.7),including a total of924cases with thecomplete data and voluntary participation in the apoE gene polymorphism detected.The surveyincludes questionnaire and clinical examination in two parts.A questionnaire survey isconducted by trained investigators to make one-to-one interviews,including commondemographic data,the smoking, drinking and past medical history, Mini-Mental StateExamination,scale of daily living activity,scale of overall deterioration,scale of lifequality,scale of geriatric depression scale.Clinical examination includes physical examination(blood pressure, height, weight, etc.) detection and blood index (biochemical, sex hormone,apoE4gene polymorphism).Results This research investigates a total of2168cases about elderly persons of55yearsand above,the average age is65.88±7.27;including893men(41.2%),1275women (58.8%);648Hui people(29.9%),1520Han people (70.1%);1229urban people (56.7%),939rural people(43.3%);in which there are457persons conforming to the diagnostic criteria of mildcognitive impairment,the morbidity rate of mild cognitive impairment is21.1%(95%CI:0.19-0.22).The morbidity rate of mild cognitive impairment in rural areas(24.3%) is higher than that in the city (18.6%);and whether in the city or in the countryside,thefemale morbidity rate(26.8%)of mild cognitive impairment is higher than themales(12.9%);with the growth of age,the morbidity rate of mild cognitive impairment presentsincreasing trend(χ~2=83.05,P<0.001);with the increase of the degree of education,,themorbidity rate of mild cognitive impairment presents increasing trend(χ~2=65.17,P<0.001).Compared the mild cognitive impairment group with control group about scores ofeach dimension of the quality of life of the elderly,there are statistical differences in thedimension of physiological function,physiological functions,physical pain and emotionalfunction;The comparison of difference between the mild cognitive impairment group andcontrol group of six genotypes and three allelic genes and gene carrying groups has statisticalsignificance; The case group of the Hui nationality and the Han nationality and the controlgroup of three allelic genes and gene carrying groups has statistical significanc, there is nodifference between the Hui of different genotypes.The non-conditional Logistic regressionresults of influencing factors about mild cognitive impairment results show that the low levelof cholesterol total, gene ApoE4carrying and low level of estradiol and neo-hombreol F aboutthe Hui nationality is the risk factors of mild cognitive impairment,the common risk factorsof mild cognitive impairment about the Han nationality and the Hui nationality are geneApoE4carrying and low level of estradiol.Conclusion①The morbidity rate of mild cognitive impairment about the people aged55years and above is higher, the primary prevention of rural, Hui, advanced age and low levelof education should be strengthened to reduce the incidence of MCI gradually and promote theelderly people healthy and happy and spend the remaining years in comfort.②The effect s ofcognitive impairment on the quality of life of the elderly are all-round, early recognition and intervention of cognitive dysfunction is helpful to improve the overall level of cognitivefunction about elderly people.③The gene ApoE4carrying is the risk factors of mildcognitive impairmen for elderly people, early recognition and intervention of the gene ApoE4carrying may prevent even reduce the occurrence and development of mild cognitiveimpairment.
