Objective: To evaluate there is the lymph nodes micrometastases in the gastric cancer whichconventional pathological diagnosis showed negative results. And to evaluate the relationship betweenlymph nodes micrometastases and clinic pathological parameters and the clinical significance of lymphnodes micrometastases of pN0gastric cancer. And to illustrate the modified radical gastrectomy+D2lymph node dissection is of great significance in the clinical treatment of early gastric cancer.Method: In this study,298regional lymph nodes which were dissected in gastrectomy specimensfrom40patients from January2006to April2012were selected. All these lymph nodes showed no lymphnode metastasis by hematoxylin and eosin(H&E) stains. All of the lymph nodes were immunostained withMonoclonal Antibody of Mouse Anti-Human Of cytokeratin7and Monoclonal Antibody of MouseAnti-Human of cytokeratin19. The microscope will be used to identify if there is lymph nodes metastasis.Results: Lymph node metastasis (LNM) was detected in26lymph nodes (8.7%) from10patients. There is16lymph nodes(from6patients) metastasis in CK7group(5.4%). While there is10lymph nodes (from4patients) metastases in CK19group (3.4%).There is significant correlation betweenMM and the depth of tumor invasion (p<0.05). There is no correlation between MM and the sex\age ofpatients and the size/location of tumor.Conclusion: There is lymph node micrometastases (LNM) in pN0gastric carcinoma. CK7andCK19are both the ideal indicators in the immunohistochemical detection of pN0lymph nodemicrometastases, and are of positive significance in guiding the surgical treatment of gastric cancer. Theradical gastrectomy+D2lymph node dissection is necessary in the treatment of early gastric cancer.
