Objective:To investigate the clinical features and the surgical treatment of endometriosisin different ages and to analyze the correlative factors of postoperative efficiency withconservative surgery.Methods:A retrospective analysis for the clinical data of179operated patients, who wereconfirmed endometriosis in different ages and were followed up, was conducted. Byregression analysis of the operation effect of factors, we screened for the related factorsfor remission of symptoms and relapses postoperatively.Results:(1) The patients’ average age was (32.7±7.6) years. All the patients were dividedinto group Ⅰ, group Ⅱ, and group Ⅲ according to their ages. There were85cases with≤30years old (group I),65cases with>30~40years old (group II), and29cases with>40years old (group III).(2) The patients who were performed mainly due to painincreased with the increases of the ages(P <0.001).The patients who were performedmainly due to infertility were found more in the groupⅠand groupⅡ than in the groupⅢ(P<0.05).(3) The patients who were treated with laparoscopic surgery or performedwith conservative surgery tended to degression from groupⅠto group Ⅲ.(4)167caseswere followed completely;138cases were performed with conservative surgery, therecurrence rate was11.6%;29patients received radical or semi-radical conservativeoperation, the recurrence rate was zero. The rates of postoperative palliation andrecurrence were not statistically significant among the three groups (P>0.05). The naturalpregnancy rates in the patients between groupsⅠand Ⅱafter conservative surgery werestatistically significant different.(5) Logistic regression analysis for the patients ofconservative surgery showed that the score in EFI was positively correlated with thepregnancy rate of endometriosis-associated infertility (OR=34.96,95%CI:1.51~808.86). Combination of postoperative medical therapy was related to the recurrence rate (OR=0.10,95%CI:0.02~0.52).Conclusion:(1) The mean age at the onset of endometriosis in the patients was earlierthan the age of medical diagnosis.(2) The younger patients were performed mainly due topelvic mass and infertility. With increasing ages, patients whose chief complaints werepain increased.(3)Patients whose age≤40are given priority to laparoscopic andconservative surgery.(4) The rates of postoperative palliation and recurrence were notstatistically significant at the different ages among the groups. The younger patients had ahigher rate of natural pregnancy.(5) The patients of high EFI score had a high pregnancyrate. Combination of postoperative medical therapy can reduce the recurrence rate.
