Objective: To compare the dynamic hip screw (dynamic hip screw, DHS), anti-rotationof the proximal femur intramedullary nail (proximal femoral nail antirotation, PFNA)and Gamma nail in treatment of intertrochanteric fracture surgical trauma, complicationsand clinical effect.Methods: A retrospective analysis of the September2009-2012in December Yan’anUniversity Hospital orthopedic admitted intertrochanteric fracture in90cases, including38cases of DHS group the PFNA group of27cases, the Gamma group of25patients.Three groups, respectively, from age, duration of hospitalization, the operation time,blood loss, fracture healing time, the incidence of postoperative complications andpostoperative recovery hip were analyzed and compared. Data were statistically analyzedto evaluate the therapeutic effect of three internal fixation of intertrochanteric fracturestreatment.Results: clinical observation and follow-up of90patients were followed up for6-18months, with an average of15.9months. The average age of three groups by statisticalanalysis after the group had no significant difference. The average length of stay of threegroups derived by statistical analysis PFNA group and the Gamma nail group, theaverage length of stay shorter than the DHS group, there is a significant difference; quitePFNA group and the Gamma nail group, the average length of hospital stay was nosignificant difference. The average blood loss of three groups twenty-two statistical analysis, the results were statistically significant differences (P<0.05). The three meanoperative time twenty-two statistical analysis, three sets of pairwise comparisons showedno significant differences P>0.05). Three groups intraoperative and postoperativecomplications after statistical analysis derived PFNA group and DHS group Gammagroup and DHS group, the difference was statistically significant (P<0.05), the thePFNA group Gamma group no statistical difference significance P>0.05). DHS group,PFNA group and Gamma group average fracture healing time was13.5±1.7weeks,respectively,12.8±1.0weeks and12.3±1.2weeks, among the three groups the averagehealing time pairwise comparison was not statistically significant (P>0.05). Evaluate thedegree of recovery of hip Harris score, three groups of early postoperative hip functionrecovery the situation PFNA group and Gamma of group was significantly better than theDHS group, forward of the hip joint function recovery was no significant difference inthe situation between the three groups.Conclusion:1. Long-term clinical effects no significant differences of the three treatment methods.PFNA treatment and Gamma nail patients early out of bed load, significantly reduced theincidence of early complications, early clinical better than the DHS group of patients.2. PFNA and Gamma Nail surgery injury, shorter operating time, less blood loss, fewercomplications, early Harris score higher than the DHS group, is the ideal method for thetreatment of intertrochanteric fractures.3. Complex intertrochanteric fracture select appropriate within fixed individual treatmentshould be based on the patient’s specific situation.
