Analysis On Clinical Cases Features Of624Patients With Colorectal Cancer In Yan’an Region

Objective: To retrospectively analyze the clinical datas of624patients withcolorectal cancer treated in Yan’an University Affiliated Hospital from January2002toDecember2011,and study the clinical cases characteristics of patients with colorectalcancer(CRC) and the variation trends in Yan’an area, further improve the diagnosis andtreatment of colorectal cancer.Methods: To retrospectively analyze the clinical datas of624patients withcolorectal cancer who were proven by histopathological examination and treated inYan’an University Affiliated Hospital from January2002to December2011. We d ivided10years into two stages equally, the first five years as Group A (310cases), the second5years as group B (314cases). We analyzed the clinical characteristics of624patientsm,include the colorectal gender, onset age, occupation, location and histological type. Allpatients had a clear pathological data. Only parts of624cases colorectal cancer patientshad a return visit information, so it failed to make the evaluation of its prognosis.624patients were divided into three groups: youth group (≤44years old), middle-aged group(45-59years old) and older group (≥60years). The location of involvement in CRCincluded rectum, left colon and right colon; left colon included sigmoid colon,descending colon ascending colon and spleen flexure; right colon included hepaticflexure, transverse colon and ileocecum. The pathological types of CRC were composedof adenocarcinoma, mucoadenocarcinoma, signet-ringcell carcinoma and other types.Results:1.Gender: Men and women constitute a ratio of1.08:1and1.18:1respectively in Agroup and B group, there was no significant difference(P>0.05); Male patients were lessthan women in young group. The gender differences of A, B groups in different parts and different pathological types were not statistically significant(P>0.05),which showed thatthe incidence of colorectal cancer site and pathological type and gender may beirrelevant.2. Onset age: The age of onset was21-78years old in A group, the average age(53.8±5.1) years old; The age of onset was23-79years old in B group, the average age(52.2±5.4) years old; the onset age of men and women in all age groups was nosignificant difference in A and B groups(P>0.05). young patients accounted for18.91%,and middle-aged and older patients accounted for81.09%in624cases of patients withcolorectal cancer.3. Occupation: The incidence of colorectal cancer was significantly higher infarmers and the self-employed people (P<0.05). The incidence of youth group andolderly group with cadres occupation increased in turn, were all significantlydifferent(P<0.05), the other occupational structure was no significant difference betweenthe three different age groups(P>0.05).4. Location: The incidence of different lesion sites in624patients were rectal cancer(59.28%), sigmoid (19.71%), ascending colon (9.29%), transverse colon (4.49%), hepaticflexure (3.04%), ileocecal portion (2.08%)descending colon (1.60%) and splenic flexure(1.28%) respectively according to a descending order. Rectal cancer of A groupconstituted significantly higher than that of group B, but the right colon was significantlylower than that of group B(P<0.05), which showed the incidence of right colon cancerincreased in the past five years. Left colon constituent ratio was significantly higher thanthat of the right colon in A group(P<0.05); and the composition of the left colon and rightcolon had no statistically significant difference in B groups(P>0.05). Rectal canceraccounted for the vast majority of colorectal cancer, which was significantly higher thanother parts of the colorectum(P <0.05). Constituent ratio of each the lesion site in eachage group of A and B groups had no significant differences(P>0.05).5. Pathological types: Adenocarcinoma accounted for the vast majority, up to88.94%, a different degree of differentiation of adenocarcinoma from high to low were moderately differentiated, poorly differentiated and well differentiated adenocarcinoma;and mucoadenocarcinoma, signet ring cell carcinoma accounted for4.97%and3.52%respectively. The constitute ratio of poorly differentiated adenocarcinoma had a clearupward trend in A and B groups(P<0.05); but constituent ratio of mucoadenocarcinomahad a downward trend(P<0.05); the constitute ratio of differentiated adenocarcinoma andmoderately differentiated adenocarcinoma had no significant changes between10years(P>0.05). The constituent ratio of Well-differentiated adenocarcinoma increased inturn in three age groups of A and B groups, which had a statistically significantdifference between the young group and older group(P<0.05); but poor-differentiated andmoderately differentiated adenocarcinoma decreased in turn in three age groups of A andB groups, which all had a statistically significant difference between the young group andolder group(P<0.05); the constitute ratio of mucoadenocarcinoma and signet ring cellcarcinoma had no significant change in different age groups.Conclusion:1.To analyze the datas of624cases colorectal cancer, colorectal cancer often occursin ages from60to79years old. The pathological type and degree of differentiation ofyoung people are worse than that of middle-aged and older patients; the constituent ratioof poorly differentiated adenocarcinoma was significantly increased in ten years, whilemucoadenocarcinoma and signet ring cell carcinoma has a downward trend.2. The incidence of colorectal cancer was significantly higher in farmers and theself-employed people, and lower in the cadres in the young group than that in themiddle-aged and older group.3. Rectal cancer accounted for the vast majority of colorectal cancer, the incidenceof right colon has a increasing trend in past5years.
