With the continuous development of economic and serious environmental pollution, environmental problems have become one of the most important factors to consider in the decision-making planning of corporate. Enterprises will gradually lead " green " into the supply chain. Besides, uncertainty demands is a common phenomenon of market demand, also is the problem that enterprise has to facing along with its development.This paper firstly reviews the related research about the issues on green supply chain management, the facility decision of green supply chain and supply chain management with uncertainty demands from the domestic and foreign scholars’ research. Based on application needs of the enterprise, we propose the facility decision problem of green supply chain with uncertainty demands. Secondly, we illustrate the related theory about the issues on facility decision of green supply chain, uncertainty demands, fuzzy mathematics and fuzzy programming. The facility decision problem of green supply chain is a preventive strategy, which mainly considers environmental protection during the decision and planning stage. Taking into account the uncertainty demands, we draw a bi-objective fuzzy programming model which based on the problem of facility location and order allocation to balance the total costs and environmental pollution of supply chain, and then solve the problem by according to the theory of fuzzy programming. Finally, we do a comparison on the changes of the facility decision of multi-level green supply chain with uncertainty demands or certainty demands through cases study; explore the effect of uncertainty demands.The results of this research expand new ideas of green supply chain management, and provide the basis for long-term decision-making planning of enterprise.(1) The needs of retailers will largely be met when the demands is acquired for certain. Otherwise, manufacturers will provide excess products to most retailers with uncertainty demands, leading to the problem about risk of excessive demand and overstock.(2) Compared with certainty demands, the total cost and environmental pollution of supply chain with uncertainty demands have increased in varying degrees. When the changes of uncertainty demands, the volatility of total cost on supply chain is stronger than environmental pollution. It means that the same amount of investment on environmental protection is more effective than simple supply chain management.(3) According to the relevant provisions of the environmental protection, we can reduce environmental pollution by limiting the total CO2emissions which is used as a hint of environmental pollution. Combined with their own different requirements on environmental protection and the total cost of supply chain, the companies can make the best decision-making and planning, referring to the facility decision program of supply chain with different demands uncertainty in this paper.
