Xiao Yao San is a famous prescription coming from Prescriptions of the Bureau of Taiping People’s Welfare Pharmacy in Song Dynasty. Xiao Yao San was first described in Prescriptions of the Bureau of Taiping People’s Welfare Pharmacy Volume Ⅸ·Treat Women’s Diseases:"It treats blood deficiency and overstrain, dysphoria with feverish sensation in chest, palms and soles, limb pain, dizziness, palpitation and flushed cheek, dry mouth and throat, fever and night sweat, inappetence and somnolence, blood heat competition, irregular menstruation, perinavel pain and chills and fever like malaria. It also treats virgin’s blood deficiency and Yin deficiency, disharmony between Ying and Wei, phlegm cough and hot flash, thinness and weakness of body, as well as epigenetic bone-steaming." In a long time since Xiao Yao San came into being, it was a special prescription for postpartum deficiency-heat. Its application had been constantly extended since Ming Dynasty and it became not just for women. Composition of Xiao Yao San is precise. Liver depression and blood deficiency, as the pathological basis for various diseases of different disciplines, are the major pathogenesis associated with Xiao Yao San. Therefore. Xiao Yao San is widely used in treating various internal, surgical gynecological, pediatric and ENT difficult diseases. This prescription can be a representative prescription regulating the function of liver and spleen, praised as "the best prescription for liver diseases". Doctors in later ages created many famous modified prescriptions on this basis. Its clinical application is becoming more extensive gradually.Traditional Chinese Medicine has a view called treating different diseases with same method. The same method can be adopted to treat different diseases because these diseases may have the same pathogenesis in their development process. Chinese traditional treatment pays more attention to the difference in pathogenesis instead of disease. Whether different diseases can be treated with the same method depends on whether there is common pathogenesis among different diseases, independent of etiology and syndrome. Only when the pathogenesis is the same, can the disease be treated with the same method. For the content, etiology and pathogenesis of Xiao Yao San, doctors in later ages had many views and discourses. Therefore, people should first get the concept of Xiao Yao San syndrome clear, and then correctly use Xiao Yao San to treat diseases. Based on much literature, this paper elaborates the cognition of doctors through the ages on Xiao Yao San and modern doctors’different cognitions and studies on Xiao Yao San, as well as its clinical application and experimental studies. In addition, this paper summarizes my tutor Li Zhenhua’s thinking of using Xiao Yao San to treat spleen and stomach diseases in aspects of etiology, pathogenesis, therapy and prescription, and then discusses the diagnosis and treatment of spleen and stomach diseases according to the out-patient medical records of my tutor.Professor Li Zhenhua has used Xiao Yao San to treat various spleen and stomach diseases on the basis of treatment based on syndrome differentiation, and obtained satisfying results as well as praises from many patients. This experience is worth learning and summarizing.
