| ObjectiveIn this thesis, through the study between early diabetic nephropathy in patients with TCM syndromes and yin-yang constitution to explore the correlation between early diabetic nephropathy TCM syndromes and yin-yang constitution, in order to provide a theoretical basis for the prevention and treatment of diabetic nephropathy.MethodSelected in line with Western medicine diagnostic standard of early DN samples, a total of111cases, divided into three groups with constitute: Yang-zang person, Yin-zang person and Yin-yang mild person. Comparison of three kinds of constitution in general, BMI, serum lipid levels glycosylated hemoglobin, traditional Chinese medicine syndromes, Western complications of the different features.Result1Early diabetic nephropathy patients have no significant difference in age, disease duration, sex ratio;2Early diabetic nephropathy patients with Yin-yang mild people compare with Yin-zang people in the number of people smoking has statistically significant (P<0.05)3Three physical crowd of early diabetic nephropathy, excess salt situations and temperaments status and preferences of the movement were not statistically different;4In patients with early diabetic nephropathy, Yang-zang people with Yin-yang mild people on the BMI index was no difference(P>0.05) Yang-zang people compare with Yin-zang people on the BMI index has significant differences (P<0.01); There are extremely significant difference between the Yin-zang people and Yin-yang mild people (P<0.001); Which Yin-zang people overweight and obesity accounted for90%of the number of Yin-zang people, and compared with Yang-zang people and Yin-yang mild people overweight and obesity there has the significant difference (P<0.01);5Early diabetic nephropathy patients with laboratory indicators show that Yang-zang people, Yin-zang people and Yin-yang mild people in Urea index has significant difference (P<0.01), which Yin-zang people urea index content was the highest (7.40±2.39); The HbAlc display Yin-yang mild people、Yang-zang people and Yin-zang people has significant difference (P<0.05), but Yin-yang mild people HbAlc value was the highest (9.59+2.22); Urinary albumin shows Yang-zang people and Yin-zang people has differences (P<0.05), Yang-zang people has the highest concentrations of urinary albumin (66.72±27.98); The rest of the laboratory parameters shows no significant difference between three types of constitutes; but three types of constitutes’TG were higher than normal (0.30-1.80mmol/L);6In early diabetic nephropathy patients, Yang-zang person appeared Yin-deficiency and dryness-heat syndromes more than the other two types person (p<0.01); Yin-zang people appeared Yin-deficiency and dryness-heat syndromes higher than Yin-yang mild people (P<0.01); Yin-zang person appeared deficiency of both qi and yin、deficiency of both spleen Qi and kidney Qi syndrome higher than the other two types in early DN (p<0.05); Yin-zang people and Yang-zang people appeared Yin-deficiency and dryness-heat syndromes、deficiency of both spleen Qi and kidney Qi syndrome with a significant difference (P<0.01). In minor symptoms, stasis syndrome and phlegm-blood stasis syndrome, the crowd of three kinds of constitutes has no differences, but those three types of constitutes patients existing higher incidence rate;7Patients with early diabetic nephropathy, three kinds of constitutes occurrence of diabetic retinopathy、diabetic peripheral neuropathy、 cerebral vascular disease、lipid metabolism disorders、diabetic footosteoporosis and infection, there was no difference; but in hypertension and cardiovascular disease, Yin-zang people compared with Yin-yang mild people, there has statistically significant (P<0.01).Conclusion1In recent years, Chinese medicine intervention in diabetic nephropathy made considerable progress, constitution of traditional Chinese medicine has made considerable development, the Chinese medicine "Preventive treatment of disease" principles according with the needs of today’s medical model, personalized prevention principle gradually get people’s attention;2From this investigation we found that early DN existing certainly correlation between TCM Syndromes and Yin-yang constitute:its TCM syndromes showed Yang-zang person appeared more Yin-deficiency and dryness-heat syndromes; Yin-zang people appeared deficiency of both qi and yin、deficiency of both spleen Qi and kidney Qi syndrome higher than the other two types in early DN; Yin-yang mild people main syndrome is not obvious, but mainly appeared in stasis syndrome and phlegm-blood stasis syndrome;3This investigation have shown that, regardless of constitutes of crowd, in the early diabetic nephropathy stage, are associated with different levels of sputum, blood stasis symptoms, and it is coincide with traditional Chinese medicine theory of long diseases numerously relate to silting blood;4Constitutions changed by the affect of innate endowment, postnatal environment, diet, habits and so on many factors; Constitutions can affect the development of syndromes in Cold and heat properties, therefore, to identify the type of the patient’s constitute, significance of personalized guidance for the prevention and treatment. We are not only from the medical aspects of disease prevention, but also from the daily diet, exercise habits, psychological factors such as disease prevention and treatment. In particular, to fully consider the dietary pattern, helps to reduce overweight and obesity constitute crowd, to reduce the metabolic disorder in patients delay early DN, and slow down the further development of DN has an important prevention and treatment significance. |
