Righting Blood Detoxification Granule On Senile Pneumonia Research On The Effects Of The X-ray Absorption

Objective:Explore the treatment method of "strengthening vital qi, clearing toxicity, removing blood stasis" which is based on the theory of lung carbuncle in Chinese medicine. Explore the applicable evaluation method for chest X-ray absorption in pneumonia. Assess the effectiveness and feature of "strengthening vital qi, clearing toxicity, removing blood stasis" granules in improving chest X-ray absorption in the treatment of senile pneumonia.Methods:The treatment method of "strengthening vital qi, clearing toxicity, removing blood stasis" which is based on the theory of lung carbuncle in Chinese medicine is discussed based on the literature research and clinical practice.The chest X-ray absorption score system for pneumonia is developed based on the review and reference of other chest radiograph score system under guidance of radiology expert.It is a randomized, double blind, multicenter, placebo controlled clinical study.304hospitalized pneumonia patients who are more than65years old were divided into treatment group and control group according to the principles of randomization. The treatment group is interfered with anti-infection treatment and "strengthening vital qi, clearing toxicity, removing blood stasis" granules treatment, while the control group is interfered with anti-infection treatment and placebo. The treatment lasts for one month, death and discharge after cure (or improvement) is the end of the treatment and the time for medication is no longer than4weeks. General information, vital signs, comorbidities, antibiotic medication, allergies, blood routine、 blood gas analysis and blood biochemistry are collected In days0, then the PSI, CURB-65and SMART-COP are assessed based on those data. The chest X-ray absorption score system for pneumonia (scoring part) is assessed in day7,21and28.Statistical analysis includes baseline data, chest X-ray absorption and the correlation among PSI, CURB-65, SMART-COP and chest X-ray absorption score.Results:1、303cases in five centers of clinical trial subjects were enrolled in this research, and finally284cases are be subsumed into PPS and statistically analyzed. The treatment group of144cases and the control group of140cases have no statistically significant differences with comparability in baseline data. There are216,214and281cases meet the items of SMART-COP, PSI and CURB-65, and236cases which are the treatment group of122cases and the control group of114cases are assessed the chest X-ray absorption score system for pneumonia.2、 There were no significant differences between the two groups in Chest X-ray absorption rate of236cases and227cases which are assessed in1week. 3、 Chest X-ray absorption rate(five degrees) of treatment group is higher than the control group in PSI level II (P<0.05)、 Chest X-ray absorption rate (two degrees) of treatment group is higher than the control group in SMART-COP1score (P<0.05). There were no significant differences among the other levels and CURB-65.4、 The correlation between the chest X-ray score and PSI is0.261,0.071with CURB-65and0.327with SMART-COP.Conclusion:1、 TCM theory of lung carbuncle can be used in treating senile pneumonia, and treatment method of "strengthening vital qi, clearing toxicity, removing blood stasis" is effective in the treatment of senile pneumonia.2、 The chest X-ray absorption score system for pneumonia is authentic and feasible.3、"Strengthening vital qi, clearing toxicity, removing blood stasis" granules could promoting the chest X-ray absorption of senile pneumonia, especially for those mild cases (PSI level II or SMART-COP1score).4、 Pneumonia chest X-ray score have no significant correlation with pneumonia severity, a single pneumonia chest X-ray evaluation can no exactly evaluate senile pneumonia severity.
