Delphi Method Was Applied To Develop Attention Deficit Hyperactivity Disorder Research In Tcm Diagnosis And Treatment Guidelines

[Objective]1. Based on Delphi method, formulate the Traditional Chinese Medicine treatment guidelines of ADHD;2. Understand and prove the pros and cons of Delphi method in formulating the Traditional Chinese Medicine treatment guidelines of ADHD.[Methods]Through full searching of the ancient and modem articles, find the ones which measure up to the Jadad standard. Use Delphi method to design the first round questionnaire in diagnosis, syndrome differentiation, treatment, prescriptions and preventive care. Send the questionnaire to40pediatrician experts and withdraw them after1month, then statistic all indexes. If the indexes measure to the mean (M)>1.5. the ratio of full score (K)>0.5, the coefficient of variation (CV)<0.5. and those are accepted by most experts retained. The other indexes deleted. After comprehensive consideration the opinions of experts, design the second round questionnaires and repeat the above method of statistical analysis. At the end of the second round questionnaires, the initial development the treatment guidelines of Traditional Chinese Medicine in attention deficit hyperactivity disorder was formulated. Hold the experts meeting to discuss the details about the treatment guidelines, and make sure the final guidelines.[Results]After literature search, expert discussions, two round questionnaires, and finalized, the first round expert positive rate was90%and the second round was100%. In the definition, clinical symptoms, diagnosis and treatment, chose the indexes which have high score. According to the experts’advices, the final treatment guidelines was formulated.1. The descriptive definition of ADHD:Attention deficit hyperactivity disorder is a relatively common childhood behavior disorders. Clinical activities too much, inattention, impulsivity wayward, poor self-control, incoordination, and weak limbs, fine motor ability, and even social adjustment disorders and learning difficulties, but intelligence is normal or nearly normal. It needs to characterize the differential diagnosis with Tourette Syndrome, childhood schizophrenia.2. ADHD is divided into five syndromes, respectively diagnosis and treatment, integrated treatment. Liver and kidney yin deficiency pattern, therapy:nourishing the liver and kidney, soothe the liver yang. The main decoction:Qijudihuang Pill. Heart and spleen deficiency pattern, therapy:nourishing the heart and relax the mind, invigorating the spleen and replenishing Qi. The main decoction:Guipi decoction and Gamnai Dazao decoction. Sputum fire trouble the heart pattern, therapy:clearing the heat and purging the fire, resolving phlegm. The main decoction:Huanglian Wendan decoction. Spleen deficiency liver wang pattern, therapy:strengthening the spleen and smooth the liver. The main decoction:Xiaoyao San. Kidney deficiency liver Wang pattern, therapy:nourishing the kidney and smooth the liver. The main decoction:Zhibai Dihuang Wan. The disease can also be treated combined with acupuncture, auricular therapy and other treatment. Particularly pay attention to children in education and guidance, psychotherapy, behavioral treatment and sensory integration training treatment.[Conclusion]Through systematic research, questionnaires, explore, and ultimately to develop the Chinese medicine diagnosis and treatment guidelines for school-age at18years of age with Attention Deficit Hyperactivity Disorder, which covered the TCM diagnosis, differentiation, and treatment. And demonstrate the important role of the Delphi method, formulated with attention deficit hyperactivity disorder in Chinese medicine treatment guidelines.
