Background and ObjectivesIrritable bowel syndrome (IBS) is one of the most common functional gastrointestinal disorders in the department of Gastroenterology, characterized by recurrent abdominal pain, abdominal distension or abdominal discomfort and changes in bowel movement habits without any detected structural disorders. Now IBS is considered as a common and worldwide disease which has a high incidence in Europe and the America. In recent years, the incidence of IBS in Asia is increasing every year. The cause and the mechanism of IBS haven’t been completely understood so far. Now most researchers consider that IBS has a multifactorial etiology, which involves abnormal gastrointestinal motility, visceral hypersensitivity, infection, immunity, psychological factor, gene and enteric dysbacteriosis and so on. Recently, researchers pay more and more attention to the role of psychological factor in the pathogenesis of IBS. A large number of IBS patients often suffer psychiatric disorders and sleep disturbance as well as intestinal symptoms, especially the patients with severe IBS. In patients with IBS, the most common comorbid psychiatric disorder is depression, followed by anxiety and somatoform disorders. At present, in China studies of parenteral symptoms of IBS patients focus on anxiety and depression. However, researchers often pay little attention to the sleep quality of IBS patientsa. Besides, there are few researches on the clinical analysis of anxiety, depression and sleep disorder in patients with IBS. This study aims to expound the psychological features and sleep quality of IBS patients by comparing the differences of anxiety, depression and sleep quality between IBS patients and healthy controls.Subjects and MethodsRandomly select53IBS patients who were admitted to the Second Affiliated Hospital of Zhengzhou University from April,2012to December,2012. IBS was defined by the Rome III criteria. At the same time, we chose50healthy volunteers as a control group. The levels of anxiety and depression between the IBS group and control group in the latest week were evaluated respectively by the Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS). And we adopted Pittsburgh Sleep Quality Index questionnaire (PSQI) to evaluate sleep quality of both groups in the latest month. All the questionnaires were completed independently by respondents. After completed, the questionnaires were collected and calculated scores by the researcher. We used the SPSS17.0statistical software to analysis the data we collect. P<0.05is considered as statistical significance.Results1. Compared with healthy subjects, IBS patients had higher scores of anxiety, depression and PSQI (45.38±6.65vs39.26±5.39;47.70±6.82vs40.96±5.28;6.51±2.34vs5.44±1.41; P<0.05). The prevalence of anxiety and depression and PSQI positive rate of IBS patients were significantly higher than those of healthy subjects (8.3%vs10%;37.8%vs12%;43.40%vs18%; P<0.05).2. Female IBS patients had higher scores of anxiety, depression and PSQI than male IBS patients (47.31±7.49vs43.57±4.39;49.99±7.13vs44.24±4.64;7.00±2.41vs5.61±1.98; P<0.05). There were no significant statistical differences in the prevalence of anxiety and depression between male IBS patients and female IBS patients. Also PSQI positive rate of female IBS patients wasn’t significantly higher than that of male IBS patients (P>0.05).3. Compared with healthy subjects, the scores of sleep disturbances, sleep quality, sleeping pills, daytime dysfunction were significantly higher in IBS patients. IBS patients with anxiety or/and depression had higher PSQI positive rate and scores than patients without anxiety and depression (P<0.05).Conclusions1. IBS patients frequently suffer more anxiety, depression and sleep disorder than healthy subjects. The major manifestations of sleep disorder are sleep disturbances, sleep quality, the using of sleeping pills and daytime dysfunction.2. Female IBS patients have higher scores of anxiety, depression and PSQI than male IBS patient3. The sleep disorder in IBS patients can be exacerbated by the state of anxiety and depression. When an IBS patient has both anxiety and depression at the same time, the sleep disorder he suffers will become more serious.
