Background and ObjectiveHuman papillomavirus (HPV) is a DNA virus, which can be widely found in nature. With highly organize and host specificity, it can cause hyperplasia of the skin and mucous. The virus can cause the verrucous and papillary lesions of the host tissue, too. The etiology of association between HPV infection and cervical cancer has been verified by a lot of scholars. High-risk HPV infection is a necessary condition and the primary factor for cervical cancer. Cervical cancer is a long-term process of gradually evolved from a pre-cancerous lesions.The risk of patients with high-risk HPV infection of cervical lesions is250times higher than without high-risk HPV infection. In recent years, the incidence of VAIN increased year by year.There are researches reported that the increased incidence is associated with many factors especially the HPV infection in the female genital tract. So the correlation between HPV and VAIN become the focus of attention in the VAIN incidence factors.Cervical cancer is one of the most common malignant tumor, which occupies a major reason for women’s death. Recent statistics show that approximately466,000new cases of cervical cancer happened per year worldwide, and our nation occupied132,000.In the past three decades, cervical cancer and cervical precancerous lesions can be detected and treated in time for the domestic popularity of the cells in a thin layer of liquid-based cervical smears joint high-risk types of HPV testing as a cervical cancer early screening method.It can also reduce the incidence of cervical cancer and make the early detection of cervical cancer,the early diagnosis and treatment of cervical cancer possible. According to the reported research, the5-year survival rate of early cervical cancer is as high as90%or more. Therefore, the amount of cervical cancer patients who underwent surgery is huge.Although the HPV and cervical cancer have a close relationship, the analysis of HPV condition in the huge people of postoperative patients with cervical cancer is rare.The management, monitoring and developing of high-risk HPV in postoperative patients cause our attention.At present, the VAIN patients who exist for cervical cancer after surgery have no principle treatment. The medication, physical therapy, laser ablation, surgery and other methods can be used depend on the factors of lesions, parts, range, age and the patients’requirements,In this paper, the preoperative outpatient follow the Second Affiliated Hospital of Zhengzhou University, high-risk HPV-DNA detection are tested on382cases of HPV-positive cervical cancer patients who had already been surgeryed, in order to explore the prognosis of postoperative patients who infected high-risk HPV, and the method of treatment for the secondary VAIN.Methods and SubjectsHigh-risk HPV testing for three hundred and eighty-two cases operated for cervical cancer treated in the outpatient service of the second affiliate hospital of Zhengzhou University dated Jan.2008through May.2011. All patients were infected by HPV before surgery.All patients were tested for high-risk HPV-DNA detection and thinprep cytologic test. The colposcopy biopsy should do in the two abnormal results. The patients who developed into VAIN are treated by5%trichloroacetic acid, laser treatment and surgery.Results1. The negative conversion rate of HPV:Kaplan-Meier curves show that the cumulative negative people are300cases in382patients. About78.5%with high-risk HPV regress in two years, the clearance median time is1.2years.2. The result of TCT:Abnormal cervical cytology results TCT (including ASCUS and foregoing) were35cases. With the time of HPV infection becoming longer, a high incidence of abnormal TCT results is observed, TCT abnormal percentage was74.3%(26/35) after2years.3. Incidence and time of VAIN:The postoperative patients with high-risk HPV infection with secondary VAIN are24cases, the rate was6.3%.The average time is38months,20.8%VAIN happens in2years, while the other79.2%occurred after2years.4. The follow-up results after treatment of VAIN:Follow-up to the end of topic, four cases of high-risk HPV are positive, three cases of TCT are abnormal, results of colposcopy biopsy are normal.Conclusion1. HPV seroconversion rate was gradually increased with time textention, most people seroconversed within2years.2. The postoperative patients with high-risk HPV infection have higher incidence of VAIN.3. Topical medications, physical, and surgical treatment are feasible and can achieve good therapeutic effect for VAIN.
