Objective:To investigate the procedure and the efficacy of treatment for severe flexion contracture in patients with total knee arthroplasties.Methods:In our orthopedics from December2011to February2013,4cases of patients with8knees, compromising of2males and2females, with severe flexion contracture of knees underwent total knee arthroplasties. Preoperatively and postoperatively, the HSS, VAS, ROM and residual flexion were followed up. And assess the satisfaction degree at last follow-up according to the questionnaire for the patients.Results:All of the patients were followed up for2months to12months. preoperatively, the average HSS was41, average flexion contracture73.5degree, average range of motion41degree, average VAS7. Two weeks after TKA surgeries, the average HSS is80.4, average flexion contracture15.6degree, average range of motion92degree, average VAS1. At the final follow-up, the average HSS was87, average flexion contracture7.7degree, average range of motion103.4degree, average VAS0.5. And4patients were pleased with the condition. Complications, taking Palsy, infection and prosthesis loosening for example, weren’t present.Conclusion:The total knee arthroplasty is an effective method to treat the severe knee flexion contracture. However, the prospective condition wasn’t definite, and more studies should be taken. In these patients, completely soft-tissue release and additional bony resection were needed. So the procedure was more complicated. After operation casting may be needed for3to28days. Postoperatively, surgeons should pay more attention to avoid the complications. And the intact rehabilitation program should be carried out to preventing from recurrence of the contracture.
