Relationship Between Helicobacter Pylori Infection And Henoch-Schonlein Purpura In Children

Purpose：To study the clinical relationship between Helicobacter Pylori (HP)infection and Henoch-Schonlein Purpura (HSP) in children and direct therapy,Observe the curative effect and prognosis.Data and Methods：Choose132children who had a clear diagnosis of HSP andhad treated in the China–Japan union hospital of Jilin University between January2005and January2013as the case group. of them,72were male and60were female,male to female ratio was1.2:1. the age range for the children with HSP was5-18years, the mean age was11.3years.course of disease was3-28days.25cases withsimplicity purpura;80cases with skin purpura and gastrointestinal symptoms, ofthem,30cases with severe gastrointestinal manifestations(abdominal pain withhematemesis and/or bloody stool);17cases with arthritis or arthralgia;10cases withrenal involvement;100cases first onset,32relapsed patients. And60healthychildren of the same age group served as the control group. Serum HP-IgG of twogroups were detected by ELISA, we also used gastroscope with Rapid UreaseTest(RUT) to check HP for80children with gastrointestinal symptoms, HP positivechildren were divided into two groups randomly: anti-HP treatment group andconventional treatment group. There is no significant difference with sex and age.We treated the anti-HP treatment group children not only used conventional therapybut also used HP eradication triple therapy: OME(Omeprazole)0.8mg/(kg.d),2times/d; Clarithromycin,15mg/(kg.d),2times/d; Amoxicillin,50mg/(kg.d),3times/d.We used anti-allergic and hormone conventional therapy only to treat theconventional treatment group children. The course of treatment was two weeks.After treatment, according to curative effect of HSP calculate the recovery rate of two groups, and record the recurrence rate of these two groups. At the end useSPSS17.0to do chi-square test, and find statistics differences, P<0.05there isstatistics difference.Result：88of132children were positive HP-IgG, Relevance ratio of positive HPwas66.67%, of them,56of80cases with gastrointestinal symptoms were positiveHP-IgG, Relevance ratio of positive HP was70.00%. and24of30cases with severegastrointestinal manifestations were positive HP-IgG, Relevance ratio of positive HPwas80.00%.24children of control group were positive HP-IgG, Relevance ratio ofpositive HP is40.00%, Two groups had difference statistical significance. Endoscopyrevealed: all80cases had changed with gastric mucosa. HP were positive in67cases,Relevance ratio of positive HP was83.75%; In anti-HP treatment group: after theanti-HP treatment, the gastrointestinal symptoms had recovered quickly. The totaleffective rate was95.45%, but in conventional treatment group with conventionaltherapy only the total effective rate was72.73%, there was significant statisticaldifferences(P <0.05) between two groups. The recurrence rate was9.09%in anti-HPtreatment group; and was22.73%in conventional treatment group, compared withanti-HP treatment group, the recurrence rate was significantly increased, but therewas no significant statistical difference (P>0.05).Conclusion：According to the study, The infection rate in132children with HSP,especially the80children with Henoch purpura was significantly higher than that innormal children, and in HSP with gastrointestinal the HP infection was significantlyhigher than the other type of HSP, that means the HP is one of the main reasoncausing or aggravating gastrointestinal symptoms. The clinical manifestations ofHenoch Purpura are consistent with the lesions of mucosa under gastroscope. Forthose children whose have HSP especially with gastrointestinal symptoms or chronicHSP should check HP as soon as possible. Eradication of HP it is good for controllingsymptoms、shortening the course of disease、and can reduce recurrence rate of HSP. Itis so important to prognosis of HSP.
