Recent years, with the population life expectancy increases, the prevalenceand the incidence of intracranial aneurysm is a clear upward trend, and elderlypatients with intracranial aneurysm incidence also increased significantly.Especially the elderly, due to the improvement of living standards prevalence ofhypertension the high increasing tendency in recent years, elderly patients withaneurysms secondary risk of rupture and easily associated with otherunderlying diseases, and are prone to causing complications and mortality thanother ages. However, with the development of microsurgical and endovascularinterventional techniques, all ages ruptured intracranial aneurysms activesurgical treatment should be under the conditions allowed, but patients withwide-necked aneurysms, and poor general condition of the body can not choosecraniotomy clipping treatment of ruptured intracranial aneurysms. Objective: The Enterprise stent in the value of complex intracranial aneurysmembolization and future applications.Datas and Methods: Bethune First Hospital of Jilin University fromDecember2009to December2012the Enterprise stent after endovascularinterventional treatment of107patients with intracranial aneurysms (109).71of them are women,36men, aged21years old-80years old (average55.86years). The unruptured aneurysm25cases,84cases of SAH onset. Course of2hours to a year. SAH Fisher classification: I16cases47cases of grade II, IIIgrade12cases, IV grade nine cases. All patients were confirmed by thecerebrovascular digital subtraction angiography check clear aneurysmdiagnosis, all patients were confirmed by cerebral angiography checks clearaneurysms, carotid artery after traffic segment48segment of the internalcarotid artery ophthalmic artery22top of the basilar artery and vertebral-basilar artery junction14, the vertebral artery and the base artery dissection9,5each of the internal carotid artery bifurcation and anterior choroidal arteryaneurysm, carotid artery clinoid segment4, the brainartery M1segment2.Dissecting aneurysm9cases; Wide-necked aneurysms (neck>4mm or tumor/neck <2)100cases of small aneurysms (<3mm)21cases,of small aneurysms(≥3to <10mm)48cases, of aortic aneurysm (≥10~<25mm)27cases, giantaneurysms (≥25mm)4cases,All patients underwent elective surgical treatment.Unruptured aneurysm preoperative3-5d given daily oral clopidogrel75mg aspirin100mg SAH patients given2~3h before anesthesia Dayton clothingclopidogrel300mg loading dose, aspirin300mg. Routine intraoperativeheparinization. The postoperative continue anticoagulation therapy (oralclopidogrel75mg, aspirin100mg) for3-6months, and then single-100mg ofaspirin taken for life.Results: The group of application109bracket4.5mm×28mm48gold,4.5mm×22mm52gold, fill in the coil of0to9. Simple stent implantation in12cases. Immediately after angiography see the aneurysm cavity contrast agent retention,patients were followed up for9cases,3-June the line head DSA examinationshowed no significantly increased aneurysm. Stent and coil embolization ofaneurysms97cases. Immediately after vascular the contrast significantlycompletely embolism86.0%8.0%subtotal embolization, incompleteembolization of6%.68cases3-12months after the first line DSA nosignificant aneurysm recurrence. In addition to the deaths in patients with morethan patients discharged Glasgow Outcome Scale (GOS) score:1fraction1cases,4fraction23cases,5fraction of83cases.Conclusion: Enterprise stent adjuvant treatment of complex intracranialaneurysm is a safe, reliable, and effective treatments.Enterprise stent useapplication prospects in the treatment of intracranial vascular intervention.
