| Objective: Through the retrospective study of two different surgicalway treatment the clinical data of children with congenital cataract,observe and analyse postoperative vision and complication.Methods:Select January2009and June2012in jilin universitythe second clinical ophthalmic hospital cataract division line ofchildren with congenital cataract surgery of26cases (42eyes). Eyecheck exclusion criteria:Inflammation of the eye,eye trauma,microc-ornea,microphthalmia,Congenital aniridia,congenital glaucoma and othereye abnormalities.According to the children operation method is dividedinto two groups: â… group of children with phacoemulsification jointposterior continuous circular capsulorhexis(PCCC) and anterior vitre-ctomy(AV);â…¡ group of children with cataracts cortex gettering technique.Less than2years old row secondary intraocular lens(IOL) implantationsurgery, More than2years old row surgery with IOL implantation at thesame time.Preoperative routine ocular examination, eliminate vitreousand retinal disease. After1week,1month,3months,1year,2yearsor3years follow-up, observing and recording the postoperative visualacuity, intraocular pressure(IOP),and anterior chamber reaction,posterior synechia, posterior capsule opacification(PCO),secondaryglaucoma,IOL clamping, membrane before IOL of such as complications.Results: In this study the42eyes of26patients,postoperative visualacuity were increased.Congenital cataract the postoperative Bestcorrected visual acuity(BCVA) in the two groups compared with preoper-ative have obvious difference(P=0.000);â… grouppostoperative BCVAcompared with â…¡ group, have obvious difference (P=0.004), â… group isbetter than that of â…¡ group;Postoperative unilateral cataract BCVAcompared with bilateral,no different (P=0.776).In1case (1eye) Aphakia glaucoma,give two local effects in drug treatment,IOP to normal range.â… group of postoperative complication:10eyes have no obvious complica-tions,14eyes occurred the1~2kinds of complications,among them,11eyes anterior chamber reaction;6eyes posterior synechia;3eyes PCO;1eye Secondary glaucoma;1eye IOL clamping;1eye membrane before IOL.â…¡ group of postoperative complications:20eyes all occurred postoper-ative complications, Among them,7eyes anterior chamber reaction;4eyesposterior synechia;20eyes PCO. The result of the postoperativecomplication were statistically analyzed:the postoperative anteriorchamber reaction X2=0.530,P=0.467; posterior synechia X2=0.155,P=0.694;PCO X2=33.478,P=0.000;secondary glaucoma P=0.545; IOL clamping P=0.545;membrane before IOL P=0.545. The statistical results show that the â… group PCO compared with â…¡ group,with obvious differences (X2=33.478,P=0.000),postoperative PCO â… group was obviously less than â…¡ group;Two groups of postoperative other complications compare,there was nodifference(P>0.05).Conclusion: Compared with the pure cortical cataract getteringtechnique,phacoecomulsification and PCCC and AV is a kind ofidealtreatment, at the same time of reducing postoperative complications caneffectively prevent the happening of the PCO,and keep the transparentoptical axis area.Suitable for IOL implantation,let children get sin-gle binocular visual function as early as possible.Postoperative joinamblyopia training,many children can get satisfactory visual function. |
