Objective:To observe and compare the results of cataract extraction with primary intraocular lens (IOL) implantation and secondary IOL implantation in congenital cataract(<2 years old).Methods:Total 38 cases (<2 years old,50 eyes) with congenital cataract, divided into group A and group B, were retrospectively studied. Group A with 16 patients (20 eyes) accepted cataract extraction + primary intraocular lens (IOL) implantation + posterior continuous curvilinear capsulorhexis (PCCC) + anterior vitrectomy (AV).While total 22 patients (30 eyes) of Group B were operated by cataract extraction + posterior continuous curvilinear capsulorhexis (PCCC) + anterior vitrectomy (AV), and then implanted intraocular lens (IOL) in phaseâ…¡after 2 years old. Complications and vision recovery were compared between these two group according to medical records and following up from 12 to 36 months after operation.Results:5 eves in group A and 22 eyes in group B complicated with irregular synechia of the pupil or iris posterior synechia (x2=11.286, P=0.001), and the difference was statistically significant (P<0.01). None of other postoperative complications, such as after -cataract, intraocular lens anterior membrane, external strabismus, decentration or capture of intraocular lens, nystagmus and secondary glaucoma, have statistically significant differences between the two groups (P>0.05). Eyesight of 14 eyes is greater than 0.1 in group A, with 5 eyes less than 0.1 and 1 eye failing to finish examination. In group B,13 eyes'eyesight is greater than 0.1 while 16 eyes' is less than 0.1 and 1 eye failed(x2=3.884 P=0.049).Difference of eyesight between the two groups was statistically significant after three years old(P<0.05), and eyesight of group A is better than the group B.Conclusion:The children with congenital cataract for younger than 2 years old are operated by cataract extraction + primary intraocular lens (IOL) implantation + posterior continuous curvilinear capsulorhexis (PCCC) - anterior vitrectomy (AV),which can effectively restore refractive interstitial transparent and reduce the degree of correct refractive error. Postoperative complications of intraocular lens implantation are not significantly increased,and eyesight is better, as compared with secondary IOL implantation.
