Clinical Efficacy Study On The Treatment Of Postmenopausal Osteoporosis With "Bajimigu Decotion"

Background:Osteoporosis (osteoporosis, OP) is a low bone mass and bone tissue micro-structure destruction, resulting in increased bone fragility fracture-prone of systemic disease. A variety of factors, including genetic factors, hormonal regulation, cytokines, lifestyle, and nutritional factors. OP occurrence is due to the activity of osteoclastic bone resorption and osteoblasts to form bone remodeling activity that constitutes the mechanism out of balance due. Postmenopausal osteoporosis is the main type of primary osteoporosis.In the traditional Chinese medicine, it is belong"Guwei, Gubi"areas, on the basis of its Pathogenesis about the liver and kidney tonic Yi jing yang xue cubic principles, according to the traditional Chinese medicine as a whole concept for the treatment of postmenopausal osteoporosis.Objective:To study the impact of osteoprotegerin (OPG) and Receptor Activator of NF-κB Ligand (RANKL), to explore the mechanism of herbal extract treatment osteoporosis at the molecular level, to lay the foundation of new formulations about Chinese medicine on the treatment of postmenopausal osteoporosis.Methods：60postmenopausal osteoporosis patients came from department of Rehabilitation, Geriatrics and Orthopaedics of our hospital, were selected from March2012to September2012, which were randomly divided into Bajimigu decotion group and Qigu capsule group, the ratio was1:1(each group was30cases). There was no significant difference of two group about age, duration, relevant factors and functional classification (P>0.05), Serum OPG, RANKL determination and comparative analysis of the two sets of symptom score and bone density, Evaluation, and adverse reactions were observed in the course of treatment before and after, Before and after the treatment course determination the serum OPG and RANKL,Compare comprehensive therapeutic effects and bone mineral density(BMD) between two group, respectively, after3months, and observed adverse reactions.Results:The Bajimigu Decotion has significant effects in symptom remission, the effective rate of therapeutic group was83.3%,while the control group was86.7%, there was no stastically significant difference between them(P>0.05).The accumulated points of symptom of traditional Chinese medicine were both statistically significant degraded(P <0.05),and there was no statistically significant difference between this two group(P>0.05).there were no untoward reactions in both therapeutic group and control group in the clinical test after3months. Both the therapeutic group and the control group cold addition the BMD and improve the levels of serum OPG, and reduced the levels of serum RANKL.Conclusion:Bajimigu Decotion cold effectively improve postmenopausal osteoporosis patient’s clinical symptoms. And there were no adverse events occurred, and it culd addition the BMD and improve the levels of serum OPG, and reduced the levels of serum RANKL.It is kind of safe medicine and be worth generalizing to clinical use.The principle of Bajimigu Decotion is tonify the liver and Kinney,YiJing YangXue. Starting from the overall concept of traditional Chinese medicine,use to treatment the postmenopausal osteoporosis.The possible mechanism is the compound of medicine has plant estrogen,improve hormone metabolism,and then promote osteoblasts and bone formation,inhibit osteoclast bone resorption,and to achieve the dynamic balance of bone metabolism.So it is an effective drug of the prevention and treatment of postmenopausal osteoporosis.
