The Research On The Efficiency Of Liaoning Province New Rural Cooperative Medical Compensation Mechanism

With the research of the DEA model deepening, the progressive development of the model extended. The model in relation to this paper is known as the "only input (output) model of DEA". The characteristic of such models is:it is from the concept of value to measure the efficiency rather than the concept of production.In this paper the measurement of the performance of the new rural cooperative medical compensation mechanism involves two DEA models, one is Discretionary Variable Model, another is Non-Discretionary Variable Model. The basic idea of Discretionary Variable Model is that all the assessed indicators are as the discretionary variables into the model to measure the performance of DMU-decision making unit to be assessed. With the means discretionary variables can be controlled and adjusted by the managers. The basic idea of Non-Discretionary Variable Model is that parts of the assessment indicators are as non-discretionary variables and other assessment indicators as discretionary variables that into the model to measure the performance of DMU to be assessed. Non-discretionary variables refer to the variables which are beyond the manager’s control. For example, in the assessment of the performance of new rural cooperative medical compensation mechanism, because of the mechanism taking the county-level co-ordination mode, thus the variables like the amount of compensation and times of the county level and below the county level for both hospitalization and outpatient for one person can be seen as discretionary variables. But the indicators of over the county level can be regarded as non-discretionary variables.The innovation of this article:First, this paper sets Liaoning province NRCMCM efficiency as performance, and builds Discretionary Variable Modeland Non-Discretionary Variable Model.Second, According to the above two models, propose the performance measurement formula of NRCMCM respectively. Third, the utilization of Discretionary Variable Model can help discover the gap between the DMU performance of the NRCMCM and its ideal state. And find the model for each DMU to learn in the process of improving the performance, by learning to them can effectively achieve their own performance improvement.The article has the following shortcomings:First, in the process of the new rural cooperative statistics data collation, of all104counties (districts and municipalities) of Liaoning province, the data of34counties (districts or municipalities) is missing that may lead to the biased conclusions.Second, the new rural cooperative medical expenses involved several elements such as the pay line, the reimbursement and the cap line, due to the data obtained can not reflect the capping line, thus the compensation for medical expenses doesn’t express reimbursement accurately. The conclusion of the paper reflects the reimbursement only approximately.Concluded as follows:First, every year the new rural cooperative medical compensation mechanism performance of Liaoning province is increasing and also the difference between the districts is shrinking.Build DV model and NDV model which contain from the year2008to the year2010210samples of the70counties (districts and municipalities). The results show that no matter in the DV model or NDV model, the average performance of the province new rural cooperative medical compensation mechanism has shown an increasing trend and the performance standard deviation decreases year by year which show that the development of the province new rural cooperative medical compensation mechanism is in good condition and the gap among counties (districts and municipalities) is shrinking.Second, the compensation ratio needs to be improved at different degrees.Improving the outpatient compensation ratio is the most important focus for the Liaoning province new rural cooperative management department, hospitalization compensation ratio is at the second place. The role that outpatient compensation factors plays on the performance improvement, maintaining and reduction is the weakest. The policy of central government new rural cooperative which emphasizes the continuous improvement of the outpatient compensation reimbursement will not significantly improve the performance of the new rural cooperative medical compensation mechanism.Third, point out that in the year2010the deficient counties (districts and municipalities) in the performance of the new rural cooperative medical compensation mechanism and its learning model.Under such circumstances that the status of going outside the county for medical treatment keeps the same,finding the year2010deficient counties (districts and municipalities) in the performance of the new rural cooperative medical compensation mechanism counties (districts and municipalities) can help identify their directions to improve their performance and give their learning models to realize the best performance.
