Study On The Equity And Efficiency Of New Rural Cooperative Medical System In Liaoning

Background: An official documents,issued by Liaoning Provincial Health and Family Planning Commission and the Ministry of Finance in March 2017,said that the New Rural Cooperative Medical System(NRCMS)and the Urban Resident Basic Medical Insurance System(URBMIS)will be merged.It is necessary to study and evaluate the current status of NRCMS in Liaoning Province,and to make a phased summary of the existing policies on the occasion of the implementation of this policy.Object: Finding out the problems in equity and efficiency,by analysis and appraisal on the status of NRCMS fund in Liaoning Province.Putting forward the suggestions for the integration of urban and rural medical insurance based on these problems.Methods: Lots of literature provide a theoretical basis for the article,description method is used to analysis of the basic situation of the system,concentration index is used to analysis the equity of the system by SPSS20.0,and lastly,the study applies DEA-BCC model and super-efficiency DEA model for calculating the efficiency of the system.Results: In terms of basic conditions,the participation rate of NRCMS in Liaoning Province has been above the national average since 2014 and reached 99%.The fund maintained steady growth,meanwhile,there are three years that the increase in expenditures is higher than the increase in funding.In the eight years,the number,cost and compensation of outpatient concentrated on primary medical institutions,the number and amount of hospitalization compensation concentrated on county hospitals,the hospitalization cost concentrated on medical institutions outside the county;as for the equity,all the concentration index was less than 0.1 almost all the time;and for the efficiency,the crste efficiency,vrste efficiency and scale efficiency fluctuated in the eight years.Meanwhile,the efficiency value in 14 cities varies greatly.Conclusions and suggestions: Overall,the NRCMS in Liaoning Province has run well based on the high coverage and the great result of grading diagnosis and treatment.Otherwise,there are still several problems,for example,the actual compensation ratio is lower than the policy compensation ratio.Both the fairness of financing and compensation were well.But there are still some problems as for the efficiency.The paper provides following suggestion.Keeping fairness,enhancing efficiency and ensuring the security of the fund.
