Objective:Exploration of pupil function changes in patients with type2diabetes mellitus and duration of diabetes, diabetic retinopathy, diabetic autonomic nerve lesions.Materials and Methods:Formulated by WHO diagnostic criteria, the diagnosis of80cases of type2diabetes in80hospitalized patients (all right eye patients), the group with type2diabetes with disease duration (1-8years in27cases,9-16years in26cases,17-25years in27cases), type2diabetes were divided into fundus examination and fundus fluorescein angiography results:no diabetic retinopathy lesions (NO-DR), simple period retinal lesions (PPDR), proliferative diabetic retinopathy (PDR). Evoked potential instrument Denmark vaidya Keypoint EMG sympathetic skin response (SSR) determination of the type2diabetes group, according to the presence or absence of diabetic autonomic nervous system damage, the type2diabetes group autonomic neuropathy autonomic nervous two groups. Random sample of50cases of50non-diabetic patients in our hospital eye clinic as a control group. C2514portable pupil measurement instrument to measure light stimulation of type2diabetes and control groups before light stimulation and dilated pupil diameter using SPSS13.0statistical analysis of relevant data in the darkroom environment.Results:1.The diameter of the pupil of the control group,a different course of disease in patients with type2diabetes.The control group before the light stimulus pupil diameter of5.13±0.28mm, the light stimulus pupil diameter was3.10±0.35mm, dilated pupil diameter was7.09±0.34 mm, duration of1-8years group light stimulus pupil diameter was4.83±0.26mm light stimulus pupil diameter of3.30±0.18mm, dilated pupil diameter was6.77±0.73mm group9-16years duration light stimulus pupil diameter was3.80±0.18mm after light stimulation pupil diameter was2.82±0.56mm, dilated pupil diameter of5.42±0.33mm, more than17-25year duration group light stimulus former pupil diameter was3.00±0.24mm and the light stimulus pupil diameter of2.90±0.43mm, Dilated pupil diameter of5.08±0.41mm. Control group and patients with type2diabetes before irradiation with dilated pupil diameter compared were statistically significant (P<0.05); greater than17-25years duration of diabetes before irradiation and after illumination pupil diameter compared to no significant change (P>0.05); light before and after dilation, normal group with disease duration1-8years group pupil diameter were not statistically significant (P>0.05), the normal group with duration of9-16years group and duration of17-25year group of pupil diameter difference was statistically significant(P<0.05)2. Control group, NO-DR, PPDR, PDR, patient’s pupil diameter comparisonThe control group before the light stimulus pupil diameter of5.13±0.28mm, the light stimulus pupil diameter was3.10±0.35mm, dilated pupil diameter was7.09±0.34mm, not associated with diabetic retinopathy in patients with diabetes before the light stimulus pupil diameterlight stimulation was5.03±0.29mm, pupil diameter3.12±0.48mm dilated pupil diameter was7.00±0.66mm, the simply of diabetic retinopathy light stimulation pupil diameter group and4.90±0.28mm, the diameter of the pupil of the light stimulus2.03±0.16mm, dilated pupil diameter of5.91±0.23mm, proliferative diabetic retinopathy light stimulus former pupil diameter was3.60±0.16mm the light stimulus pupil diameter was2.52±0.32mm, dilated pupil diameter5.73±0.41mm. Control group with type2diabetes group before irradiation with dilated pupil diameter compared were statistically significant (P<0.05); the proliferative diabetic patients pupil diameter light stimulation before and after light stimulation compared, no significant statistical significance (P>0.05); the light stimulus not statistically significant (P>0.05), compared to the control group with no diabetic retinopathy and patients with simple diabetic retinopathy pupil diameter, the control group with the proliferative phase of diabetic retinopathy compared to the diameter of the pupil of the lesions were statistically significant (P<0.05); after dilation, the diameter of the pupil of the control group with non-diabetic patients with retinopathy not statistically significant, with simple and proliferation period in patients with diabetic retinopathy pupil diameter compared statistically significant (P<0.05).3. Control group, type2diabetes without autonomic nervous lesions in patients with pupil diameterControl group before the light stimulus pupil diameter of5.13±0.28mm, the light stimulus pupil diameter was3.10±0.35mm, dilated pupil diameter was7.09±0.34mm in patients with autonomic neuropathy light stimulus pupil diameter of4.98±0.29mmlight stimulus pupil diameter of3.03±0.23mm, dilated pupil diameter was6.12±0.53mm, in patients with autonomic neuropathy patients before the light stimulus pupil diameter of3.32±0.16mm in patients after light stimulation pupil diameter of2.95±0.46mmafter dilated pupil diameter of5.01±0.40mm.Accompanied by autonomic neuropathy, diabetics and non-diabetics with autonomic neuropathy diameter of the pupil dilated with light stimulus ago compared to before were statistically significant (P<0.05); accompanied by autonomic neuropathy in patients with pupil diameter in no significant change (P>0.05) after the light stimulus light stimulus; compared to the diameter of the pupil of the control group and patients with autonomic neuropathy light stimulus was not statistically significant (P>0.05), but statistically significant after dilation (P<0.05)Conclusion:1. The longer the duration of diabetes, type1and2, the greater the impact on the pupil function.2. Diabetic retinopathy may precede pupil dysfunction.3.Diabetic patients without autonomic neuropathy also can experience pupil function abnormalities, the autonomic neuropathy pupil is harder to spread, and the pure light reflex will be later.
