Clinical Effect Of External Fixator Combined With Limited Osteotomy In The Genu Varum Deformity

Objective:To investigate the advantage and clinical effect on treatment of external fixator combined with limited osteotomies in the genu varum deformity.Metheds:Form January2004to February2010,40patients with genu varum deformity were performed both external fixator fixed and limited osteotomy in two hosipitals (Longgang central hospital in Shenzhen and Huakan hospital in Wuhan),13patients were male,cases,27patients were female, their age ranged from8to29years, an average age was18years. There were six deformity reasons, physical sexual bone diseases (rickets,24cases), osteopsathyrosis (6cases), achondroplasty (4cases), osteomyelitis sequela (2cases), trauma sequela(2cases), epiphyseal dysgenesia (2cases). All parameters were recorded at the preoperative and postoperative2weeks, after two years of knees spacing, preoperative related indicators and postoperative related indicators, including tibiofemoral Angle, tibia Angle, tibiofemoral joint spacing, the functional score index of knee, were evaluated.Results:Each type of interval space between both knees were decreased from preoperative12.45±6.47cm to4.91±4.08cm after two weeks(P<0.01), and3.97±3.86cm after two years(P<0.01). Tibiofemoral angle were reduced from preoperative188.23±3.65to171.65±1.03after two weeks(P<0.01), and170.65±1.49after two years(P<0.01). Shin Angle were decreased from preoperative97.63±3.48to88.33±2.39(P<0.01) after two weeks, and86.83±1.36after two years (P<0.01). Tibiofemoral joint joint spacing were decreased from preoperative2.48±0.84to3.97±0.75(P<0.01)after two weeks, and5.27±0.75after two years (P<0.01). Knee functional score alse were decreased from preoperative59.05±5.42to82.80±4.27(P<0.01)after two weeks, and85.83±3.90after two years (P<0.01).Conclusion:Combined external fixator according to osteotomy parts and osteotomies in different ways can osteotomies proximal and distal flexible assembly into three dimensional fixed. accord with, In according to osteotomies and joint need cross-cultural fixed and fixed time adjust fixed, biomechanical fixed and stable strong can avoid stiff joints. Combined external fixator and limited osteotomies need not peel off the subcutaneous tissue and periosteum and can avoid damage of osteotomies parts for blood. External fixator combined with limited osteotomies have satisfied curative effect for genu varum deformity.
