Objective:To Investigate the pathogens distribution, pathogens drug resistance and the clinical characteristics in patients with healthcare-associated pneumonia(HCAP).In Zunyi Medical College Affiliated medical hospitalized patients. Methods:A Retrospective observational study was conducted in the patients with HCAP who were hospitalized from January2007to December20011.The community acquired pneumonia(CAP) as control group. The baseline characteristics, pathogen distribution, pathogens drug resistance, antibiotic regimens and outcomes were reviewed. Results:A total of pneumonia76cases Patients, accounting for50%of all the hospitalized patients with HCAP. The HCAP mean age was65.7±14.3years old, more basic diseases and clinical manifestation was atypical. The Frequently isolated pathogens were A.baumannii, P.aeruginosa,S.aureus and E.coli strains. The Gram negative bacilli showed higher resistance to Amoxicillin, Ceftriaxone sodium, Levofloxacin, Gentamicin, Amikacin,Tobramycin and SMZ-TPM easy generation resistance. The gram positive cocci showed lowest resistance to Vancomycin and Linezolid (0%),the other antibiotics Severity drug resistance. Compared with community acquired pneumonia, the gross resistance higher (47.1%and31.1%respectively, p<0.05).HCAP common pathogens of resistance, not reasonable use of antibiotics, length of hospital stay and hospitalizd charge are higher than the CAP.Conclusions:This Study shows that HCAP.Which is more commonly found in the elderly, especially with comorbidity, clinical manifestation atypical. The major pathogen is A. baumannii,P.aeruginosa,S.aureus and Escherichia coli strains. HCAP with more risk factors, easy to have the multiple drug-resistant pathogens, need more reasonable selection of antibiotic treatment strategies,as far as possible cover multiple antibiotic resistant bacteria pathogens. Length of hospital stay, hospitalizd cost and has mortality higher than CAP.
