Objective:This study was designed to observe the preemptive analgesia efficacy of flurbiprofenaxetil during postoperative analgesia in patients undergoing modified radical mastectomyand to explore the preemptive analgesia efficacy of flurbiprofen axetil on T-lymphocytesubsets and NK, in order to provide the reference for breast cancer patients ofpostoperative analgesia.Methods:40patients (ASA I or II) undergoing elective modified radical mastectomy undergeneral anesthesia were randomly categorized into2groups with20cases each,flurbiprofen axetil group and control group. Patients in flurbiprofen axetil group receivedintravenous flurbiprofen axetil50mg(5ml)at15minutes before surgery, while patients incontrol group received intravenous physiological5ml at15minutes before surgery. Thedrugs of anesthesia induction and maintain were the same. All patients receivedpatient-controlled intravenous analgesia (PCIA) postoperatively. The scores of visualanalog scale(VAS) was used to evaluate the degree of the postoperative pain at2h、6h、12h、24h and48h. Record the total drainage from the chest and armpit, and the adversereactions in patients with the situation. The expressions of T-lymphocyte subsets(CD3~+、CD4~+and CD8~+) and natual killer cells(CD16~+CD56~+) were measured by flow cytometryat five time points: before anesthesia, at2h after incision, the first day、the second day andthe fifth day after operation.Results:1.There were no significant difference between the two groups with age, height,weight, surgery duration and blooding loss(P>0.05)(table1). Postoperative analgesiacomparison:the scores of VAS had significant difference between the two groups at2h、 6h and12h postoperatively(P<0.05), but the scores of VAS had no significant differencebetween the two groups at24h and48h postoperatively(P>0.05)(table2).There were nosignificant difference between the two groups with drainage volume from the chest andarmpit(P>0.05).Two patients in flurbiprofen axetil group experienced nausea andvomiting,while only one patient in control group experienced nausea postoperatively, andthere was no significant difference between the adverse effects rate of the two groups (P<0.05)(table3).2.There was no significant difference between the baselines of CD3~+、CD4~+、CD8~+、CD4~+/CD8~+and CD16~+CD56~+of the two groups. CD3~+、CD4~+、CD4~+/CD8~+andCD16~+CD56~+of the two groups began to decrease at2h after incision, and be lowest at thefirst day after operation, then bengan to return at the second day after operation, there wassignificant difference compared with their baselines(P<0.05). Until the fifth day afteroperation, CD3~+、CD4~+、CD4~+/CD8~+returned to their baselines, there was no significantdifference compared with the baselines(P>0.05), but CD16~+CD56~+was still lower thanthe baselines, and there was still significant diefference compared with the baselines(P<0.05). Compared with the control group, CD3~+、CD4~+and CD4~+/CD8~+in flurbiprofenaxetil group decreased less only at the first day after operation(P<0.05), whileCD16~+CD56~+at both2h after incision and the first day after operation(P<0.05). CD8~+oftwo group had no significant difference at different time points(P>0.05).Conclusion:1. Compared with the control group, preemptive analgesia effects of flurbiprofenaxetil in patients undergoing modified radical mastectomy is exact, ease to use, and lessadverse reaction.2. Preemptive analgesia of flurbiprofen axetil had protective effect on T-lymphocytesubsets and natural killer cells in patients undergoing modified radical mastectomyperioperative period, there, can protect patients’ immune function...
