Background: Chronic obstructive pulmonary disease(COPD)is a diseasecharacterized by airfiow limitation, not completely reversible restrictedairflow.Many studies show that inflammation is a central part of thepathogenesis of COPD.Diabetes is a systemic metabolic syndrome,caused bythe genetic and environmental factors.Recent researches show that DM is achronic inflammatory response disease.Chronic obstructive pulmonary diseasewith type2diabetes is an increasingly common problem in china.but themechanism of it is still dimness. So the study of the mechanism has the vitalsignificance to clinical practice.Objective:To detect the change of serum IL-10、IL-18in patients with chronicobstructive pulmonary disease with type2diabetes, To explore the relationshipbetween IL-10、IL-18with chronic obstructive pulmonary disease and type2diabetes mellitus (T2DM)),provides new ways for the prevention and control ofCOPD.Methods:1.We observed a total of80people who were chosen in our internalFirst Affiliated Hospital of JiLin College of hospitalization outpatient serviceand hospitalized patients during December2011and September2012.Patients with acute phase COPD(woman10, man10)、 normal control(woman10,man10)、patients with type2diabetes(woman10, man10)、patientswith acute phase COPD with type2diabetes (woman10, man10)were enrolledin the study. Pulmonary function were detected at admission.2. All participants were tested lung function index and taken median cubitalvein blood. Then the blood samples were centrifuged and collected and thenserum was stored at-70°C. for the preparation of uniform testing the IL-10、IL-18.3. We took serum specimen from patients Who were at COPD acute phaseand COPD acute stage with diabetes mellitus after treatment, according tomethod2and measured their lung function.4.The serum of IL-10、IL-18were measured according ELISA method.Results:1.The result of IL-10Serum IL-10:the serum level of IL-10in the acute phase COPD the stablephase COPD,the acute phase COPD with type2diabetes, COPD acute stagewith diabetes mellitus after treatment, the type2diabetes group and the controlgroup.3.45士2.6,8.41士1.22,1.83士0.3,3.67士0.18,8.41士1.22,17.56士1.13.It has significant difference in each group (P<0.01).2.The result of IL-18Serum IL-18:the serum level of IL-18in the above group.422.75士17.5,276.12士10.1,493.3士13.4,350.8士10.8,276.12士10.1,48.8士2.73.It has significant difference in each group (P<0.01).3.The result of lung function indexFEV1.0%:the level of FEV1.0%in the above group.57.16士9.6,69.81士6.1,45.48士4.55,58.27士3.5,67.81士6.1,83.41士11.45.FEV1/FVC: the level of FEV1/FVC in the above group.54.22士12.65,67.87±5.79,46.42士3.5,55.74士2.4,74.52士0.8,90.83士10.DLCO/VA:the level of the serum level of DLCO/VC in the above group:67.28士10.13,87.01士5.9,58.32士3.7,75士2.55,80.64士3.48,110.41士8.1.4.The level of IL-18in serum of the acute phase COPD and the stablephase COPD was inversely cor-related with FEV1.0%(r=-0.415、P<0.01,r=-0.324、P<0.01)The level of IL-10in serum of the acute phase COPD and thestable phase COPD positive cor-related to FEV1.0%(r=0.672、P<0.01,r=0.432、P<0.01).(4)Conclusions:(1)The levels of serum IL-10in the acute phase COPDgroup are lower than the stable phase COPD group and the healthy controlgroup.The stable phase is higher than the acute phase.It has significantdifference (P<0.01)in these three groups. The IL-18in blood are higher in theacute phase COPD patients and the stable phase COPD patients than thehealthy control group.The stable phase is higher than the acute phase.It hassignificant difference (P<0.01).(2) The levels of serum IL-18are higher in theCOPD with type2diabetes group than COPD group and type2diabetes.It hassignificant difference (P<0.01).The IL-10in blood are lower in the COPD with type2diabetes group than COPD group and type2diabetes.It has significantdifference (P<0.01) It showed that IL-18,IL-10may participate in the processof COPD with type2diabetes.It can be used as judgement index of acuteperiod of COPD with type2diabetes.(3)IL-18and IL-10were correlated withpulmonary function,which showed that IL-18,IL-10may take part in the degreeof airflow obstruction degree and reflect the serious degree of napulmoryfunction degression.(4)The dispersion and ventilatory function in the COPDwith type2diabetes are obviousely lower than the COPD group and type2diabetes group.It has significant difference (P<0.01). It showed that diabetescan damage the lung function.
