The Clinical Study Of Short-term And Medium-Term Outcomes Of Domestic-made Rapamycin Drug-Eluting Stents In Treatment Of Ischemic Cardiomyopathy

Objective:This study proposed by comparing the two kinds of methods for ischemic cardiomyopathy, follow-up study into Domestic-made Rapamycin Drug-Eluting Stents and simple application of the drug in patients with the same period before and after treatment and Short-term and medium-term outcomes difference between them. To investigate the effects and prognosis of Domestic-made Rapamycin Drug-Eluting Stents on the treatment of heart failure derived from ischemic cardiomyopathy.Methods:In January2009to may2011,the patients lived in Department of The Second Medicine,The Second People’s Hospital of Guilin, who were diagnosed as coronary heart disease with heart failure according to the result of coronary arteriography and<Internal Medicine>(The seventh edition) diagnostic criteria, a total of68cases of observation. The patients were divided into2groups:do Domestic-made Rapamycin Drug-Eluting Stents placement and drug treatment for the treatment group, a total of32cases, only do conventional treatments as control group, a total of36cases. To compared the changes of clinical symptoms,signs(Including chest pain,bosom frowsty,shortness of breath), New York heart function classification (NYHA) and the change of heart ultrasonography(IncludingLeft ventricular ejection fraction,Left ventricular fractional shortening,Left ventricular end-diastolic internal diameter) in admission, the treatment of three months, six months and one year of two groups. Comparing the two groups of patients,in the treatment of a year turned their mortality.Results:The average times in hospital of interventional revascularization group is longer than pure standardized medication group; Average age, gender, medical history, laboratory examination statistics have no obvious difference (P>0.05); Two groups of patients in the numbers of lesion coronary arteries, the Narrow degree, blocked lesions (for acute myocardial infarction) and the admission of the change of heart ultrasonography(IncludingLeft ventricular ejection fraction,Left ventricular fractional shortening,Left ventricular end-diastolic internal diameter) has not seen the difference obvious (P>0.05). Two groups of patients after treatment in three months after bosom frowsty, chest pain, shortness of breath, cardiac dysfunction symptoms are improved, but found no statistical difference (P>0.05), but in the treatment for6months and one year, stent placement treatment group bosom frowsty, chest pain symptoms improved significantly better than control group (P<0.05); The treatment of a year, the stent placement treatment group shortness of breath symptoms improved significantly better than control group (P<0.01). the percentage of people whose NYHA Ⅲ-Ⅳin admission and three months later have no change (P>0.05), but after six months and even a year changed obviously (P<0.05).Have been treated for three months,the Left ventricular ejection fraction(EF) of interventional revascularization group have enhanced obviously than pure standardized medication group (P<0.05), even more,the Left ventricular fractional shortening(FS) and EF have enhanced after six months and one year (P<0.05),and the Left ventricular end-diastolic internal diameter (LVEDD) have minished (P<0.05). In a follow-up, stents placement treatment group all major clinical event rate is low control group (P<0.05), including all cause death, all heart source sex pieces, all cardiac to be in hospital, angina pectoris readmission, heart failure and increase in hospital are again index difference (P<0.05). The control group4deaths in3patients died of heart failure,1patients died of cerebrovascular accident. Two groups of patients in the treatment of a year later, the control group no patients do coronary arteriography review; Stent placement therapy group was19routine coronary angiography review, all did not happen stent restenosis.Conclusion:1. Domestic-made Rapamycin Drug-Eluting Stents placement in the improvement of ischemic cardiomyopathy, the clinical symptoms and cardiac function has obvious function.2. Domestic-made Rapamycin Drug-Eluting Stents placement for ischemic cardiomyopathy effect significantly better than simple drugs.3. Domestic-made Rapamycin Drug-Eluting Stents placement to improve the Short-term and medium-term outcomes also has certain value.
