Empirical Study On Utilization Of Essential Medicines Before And After National Essential Medicines Systems In Tianjin

In view of the re-establishment of NEMS (National Essential Medicine System) in our country, Tianjin has its own EML (Essential Medicine List) containting supplementary medicines based on NEML (National Essential Medicine List). This study is to evaluate implementation of the NEMS from the viewpoint of utilization of the Essential Medicines (EMs), and to describe the use of EMs and Cardiovascular EMs, which could offer empirical evidences for management of using EMs and adjustment of supplementary medicines.A retrospective analysis on the varieties and costs of EMs of Tianjin, including NEMs (National Essential Medicines) and supplementary medicines, was conducted by trend analysis method, the elastic analysis method, the key analysis and descriptive statistical analysis. The medicines of interest were extracted from the "2010Tianjin basic community health services administration directory (Trial)", and the data were then obtained from Tianjin2003-2010inpatient medical insurance for urban workers.The primary results of this study obtained as follows:①East China district has supplemented more kinds of medicines, In particularly Shanghai has the most followed successively by Jiangsu and Anhui.②Less than80%usage of EMs directory varieties throughout all the survey years, and less than30%of annual drug costs. Usage costs proportions of EMs in different MIs (Medical Institutions) ranked as first-level, second-level and third-level. However, it was opposite for the categories used.③From year2003to2010, first-level MIs used124~196NEMs and103~159supplementary medicines, which showed a upward trend. The cost percentage of NEMs in all medicines costs was13.0%-32.2%, which decreased along with years, and it was19.9%-31.9%for supplementary medicines, which increased along with years in contrary.④After the establishment of NEMS, hospitalization cost decreased, in which the supplementary costs increased yet. Hospital peoples proportions in first-level MIs decreased in2010.⑤The important classes of category and cost were Motazomins and Internal Medicine, and in Internal Medicine remove stasis agents was the most.⑥The utilization of cardiovascular medicines accounted for1/3of all EMs at an average level in the primary MIs. For cardiovascular medicines of EMs, the supplementary cost averaged54.3%for all supplementary medicines. More youngsters, more man, and more retirees tended to utilize these medicines after the implementing of NEMS, as well as more circulation system diseases.The primary conclusions of this study obtained as follows:①Provincial Essential Medicine Supplementary List is associated with economics and characteristics.②Usage of EMs is low, but it is better after NEMS was implemented.③Tianjin supplementary medicines play a dominant role in the use of the EMs, which demonstrates the reasonable aspect of the supplements, consistent with the need of the Tianjin medication.④The basic-level MIs don’t make a shunt role in receiving patients as soon as the implementing of NEMS, hospitalization cost decreases.⑤After the implementation of the NEMS, the rational use of cardiovascular medicines of EMs in curing cardiovascular disease is promoted.
