Objective:Research the current situation of the antibacterial medicines in China’s National Essential Medicines List, and the potential problems existed in the China’s management policy of antibacterial medicines, in order to supply some references about selecting and adjusting the antibacterial drugs included in the National Essential Medicines List and Supplement for Essential Medicines List, and to offer some data for the major antibacterial resistance surveillance work, moreover, to put forward some advises for the clinical using and management of the antibacterial medicines in China.Methods:(1) Quantitative analysis:By using the SPSS 19.0 statistical software, quantitatively analyze the increased amount and rate of the antibacterial medicines in NEML-2012 compared to that of the drugs in NEML-2009, and the differences between the antibacterial medicines and the other major classes of drugs in NEML-2012, and the growth amount of these classes of drugs were descriptively analyzed and compared; and then, quantitatively analyze the overlap ratio of the antibacterial medicines doubly included in the AML-D and the AML-HN, and the distribution of the same drugs in the different groups, and so on.(2) Comparative analysisBy making the charts, compare the similarities and differences among the NEML-2009, NEML-2012, SEML and EML-WHO, and find out the medicines included in the SEML and EML-WHO while excluded in the NEML-2012 and so on; compare the similarities and differences in terms of the included drugs and their management grades between the AML-D and the AML-HN, and so on.(3) Cross-sectional analysisBy using the chi-square test, a municipal tertiary level 1st class hospital in Henan province was chosen as the research model and the drug susceptibility data to the major pathogenic bacterium in 2010 and 2013 as the two research sections, the differences at the antibacterial medicines’susceptibility were analyzed.Results and Conclusion:Essential medicines are the footstone of clinical use. The antibacterial medicines included in the NEML-2012 have rich number and kinds, great increase, and good overlap with EML-WHO, and more, the Supplement for Essential Medicines List plays a important role in enriching the kinds of antibacterial drugs, supporting the choosing of national essential medicines, these are safeguarding the antibacterial drugs’clinical use very well; The Clinical Management of Antibacterial Medicines guides these drugs’clinical use. The classifying management of the antibacterial medicines in AML-HN is reasonable, and there are seven drugs such as cefprozil made grade promotion among the AML-HN, which in control of pathogenic resistance have attributed a positive effect, however, some drugs, included a few essential medicines, also appeared the resistance increase to some main pathogens. In order to further promote the rationality of the chosen antibacterial medicines in China’s National Essential Medicines List, and to improve the clinical management and use about these drugs, in these paper the author propose:the three new drugs selected in NEML-2012, benzathine benzylpenicillin, doxycycline and metronidazole, could be analyzed the general applicability in China; The safety, effectiveness and the general applicability of dirithromycin could be researched more; the drugs include in SEML and/or EML-WHO, such as cefotaxime and spectinomycin could be evaluated in detail; The Province Antibacterial Medicines Management List could bring in more drugs which are beyond medical insurance but using frequently, and be appropriate to expand the scope of the antibacterial medicines chosen to promote the reasonable selection for medical institutions and avoid drugs use too intensive in range, the drugs’ safety and the bacterial resistance should be informed timely, the potential multidrug-resistant organism should be intensively monitored and medical care personnel should checking actively, in order to reduce the occurrence of multi-resistant bacteria infection and bacterial resistance as much as possible. In addition, the author expect that Province Supplement for Essential Medicines Lists and the Province Antibacterial Medicines Management Lists could be updated timely, and they could have better and more connection with National Essential Medicines List, so to fully ensure the safety, effectiveness, reasonableness and economic burden for people’s drug use.
