| Objective:This experiment examines the concentration differences of serum IGF1, sICAM1andsVCAM1in aneurysmal subarachnoid hemorrhage. And make a linkage with clinicalmeaning. Methods:The specimens were gathered from the Department of Neurosurgery, Tongji medicalcollege hospital in Huazhong University of Science&Technology. Fourteen (14) patientsaged18years or over admitted to Tongji medical college hospital within3days after SAHfrom Dec.2011to Mar.2012. After confirmation of SAH by Brain CT. DSA examinationfound intracranial aneurysms. No other parts of the associated injuries. The exclusioncriteria were: unknown exact bleeding day, bleeding more than three days before inclusion, with any brain surgery after SAH, moribund state of the patient. Comparison was madewith a group of9healthy adults (Control group). Cubital vein blood samples10ml werecollected from the first to third mornings and the second morning after aneurysm surgery incase group. In control group, the corresponding blood samples were collected from the firstto third mornings. Blood samples were left to coagulate at room temperature1020minutes,then centrifuged for20minutes (2,0003,000Rev/min). Serum was kept at80℃.Additional routine laboratory parameters, such as electrolytes, were collected on a daily basis. Enzyme linked immunosorbent assay(ELISA) was used to measure the concentrationof IGF1, sICAM1, sVCAM1. To use Statistical analysis was performed using SPSSsystem package. We used X±Sto represent the results. Then analysed the concentrationand discussed the clinical significance.Result:1. Comparing case group with control group, concentration in the blood serum ofIGF1within3days after SAH, concentration has no statistically significance.(d1: case group17.22±7.01ng/ml, control group21.74±13.95ng/ml, P value>0.05d2: case group18.02±5.27ng/ml, control group26.70±20.79ng/ml, P value>0.05d3: case group18.13±6.49ng/ml, control group27.02±21.20ng/ml, P value>0.05)2. Comparison of case group in first days after SAH with after aneurysm surgery. The concentration in the blood serum of IGF1has statistically significance.(The first days after SAH17.22±7.01ng/ml, after aneurysm surgery29.02±7.67ng/ml, Pvalue <0.05)3. Comparison of case group before aneurysm surgery with after aneurysm surgery. The concentration in the blood serum of IGF1has statistically significance.(Before aneurysm surgery17.59±6.09ng/ml, after aneurysm surgery29.02±7.67ng/ml, Pvalue <0.05)4. Comparison of case group in3days after SAH with after aneurysm surgery.Average of concentration in the blood serum of IGF1has statistically significance(In3days after SAH18.09±6.07ng/ml, after aneurysm surgery29.02±7.67ng/ml, P value<0.05) 5. Comparing case group with control group, concentration in the blood serum ofsICAM1within3days after SAH, concentration has statistically significant.(d1: case group839.58±424.64ng/ml, control group429.28±61.91ng/ml, P value <0.05d2: case group934.21±394.99ng/ml, control group418.14±59.88ng/ml, P value <0.05d3: case group952.22±390.81ng/ml, control group391.15±95.67ng/ml, P value <0.05)6. Comparing case group with control group, concentration in the blood serum ofsVCAM1within3days after SAH, concentration has statistically significant.(d1: case group65.68±12.25ng/ml, control group51.52±11.69ng/ml, P value <0.05d2: case group67.19±12.75ng/ml, control group45.92±15.19ng/ml, P value <0.05d3: case group76.56±17.88ng/ml, control group51.08±16.91ng/ml, P value <0.05)Conclusion:1. Comparison of case group with control group. No statically significant differencein blood serum IGF1was found between case group and control group for the first3consecutive days after SAH. The results may reflect diminished acute pituitary function.2. Comparison of case group before aneurysm surgery with after aneurysm surgery. The concentration in the blood serum of IGF1has statistically significance. After promptedto lift the aneurysm and hematoma to stimulate, IGF1has a positive effect on therestoration and protection of brain.3. The concentration of sICAM1and sVCAM1associated with aSAH shows thathigher concentration concurrently leads to worse inflammatory response... |
