Objective:We aim to study the association between the TCM Syndrome elements differentiation and its related factors in Type2Diabetics Patients with diabetes related distress.Subjects and methods:According to the inclusion criteria,303cases of type2diabetes mellitus patients were chosed as research objects from hospital Endocrinology, January2011-January2012. All subjects were accessed by Diabetes Distress Scale(DDS) and divided into253cases of type2diabetes Distress group and type2diabetes non-Distress (control group)50cases. Using the method of differentiation of syndrome elements created by Professor Zhu Wenfeng, with standardized collection of the diagnosis informations, to study the TCM risk factors and distribution of syndrome elements. And then we attempt to explore the correlation of DDS scores with Western medicine, clinical physical and chemical indicators and common TCM syndrome factor.Results:1.303cases of type2diabetes psychological assessment:83.5%of diabetic patients suffering from diabetes-related distress, including8.6%of patients with high diabetes distress.2. Type2diabetes distress group with the control group compared to the duration of diabetes, education level, HbA1C, complications were significantly different (P <0.01); gender, fasting plasma glucose, TG and cholesterol (CHOL), etc. compared with the control group, statistically significant (p<0.05).3. Syndrome factor distribution:the disease of type2diabetes distress syndrome factor mainly in liver, kidney; disease syndrome factor Yin, Qi stagnation was most common, and the remaining wetlands, qi deficiency, blood stasis, deficiency of blood, heat, Yang. Conclusions:1.303cases of type2diabetes psychological assessment:83.5%of diabetic patients suffer from diabetes-related distress, including8.6%of patients with high diabetes distress. Thus the psychological assessment is particularly important early on diabetes.2. Women, longer duration, poor glycemic control, lipid metabolism disorders, the low levels of education and more complications in patients with type2diabetes may be more susceptible to diabetes-related psychological distress.Should take note of the multiple risk factors clinically to assess the early diagnosis of diabetic Distress, and take appropriate psychological counseling to prevent further deterioration of the disease.3. DDS scores show a positive correlation with the female sex, disease duration, HbA1C, fasting plasma glucose, complications while a negative correlation with educational level, suggesting that the female sex, disease duration, HbA1C, fasting plasma glucose, complications, low education level are independent risk factors.4. Liver, kidney are the main disease-organs of diabetes distress and qi stagnation, yin deficiency are the main pathological factors. Deficiency-excess complex pattern is the main pathogenesis characteristics.5.The presence of pathological changes in liver, spleen, kidney, qi stagnation, yin deficiency, blood deficiency, such as susceptibility to diabetes distress, especially the liver Qi stagnation, kidney and spleen yin deficiency-based disease risk factors.6. Suggests that the clinical use of psychological counseling and the use of liver qi stagnation, Ziyin fill the splenorenal TCM treatment intervention in order to better glycemic control, reduce or delay complications development and then improve the quality of life of patients.
