Purpose:Sebaceous carcinoma is a rare clinical disease which is even more rare in genitourinary system., By immunohistochemistry, diagnosis, treatment analysis and review of the literature of one case of scrotal sebaceous gland carcinoma,we discuss the latest results and the problems of present study, in order to help clinicians to understand the disease.we hope to provide a reference of this disease in clinical work for early detection and a reasonable diagnosis and treatment, to reduce the misdiagnosis of the disease。Materials and Methods:Through Pathological specimens of one case of scrotal sebaceous carcinoma we examined by immunohistochemistry and special staining we confirm the diagnosis. Then we collect and translate the relevant literature.Result:Pathological specimens examined by immunohistochemistry and special staining show of cytokeratin CK (+), epithelial membrane antigen EMA (+), special staining PAS (+). we confirm the diagnosis of scrotal sebaceous carcinoma. Conclusion:Sebaceous carcinoma is a rare malignant tumor, more common in orbit around. It rare occur in other parts of the body. It can also be one kind of a variety of skin tumors in Muir-Torre syndrome.Clinical manifestations of the sebaceous carcinoma with diversity, clinicians must be vigilant and note that in order to avoid delaying the best timing of treatment, leading to tumor metastasis to life-threatening. So far, the tumor is still lack of a clear differential diagnosis, clinical diagnosis is prone to misdiagnosis. With the continuous development of immunohistochemical techniques and molecular biology research and the continuous development of new tumor markers. we expected to finally reveal the pathogenesis of the sebaceous carcinoma, and provide a new approach to the prevention and treatment. For the more obvious racial differences of sebaceous carcinoma, this research has rarely been reported in our country. Therefore, the study task is more difficult and great significant to medical workers. Treatment of malignant tumors is life. Even if under the premise to we ensure that the margins were negative with complete resection of the tumor, it also don’t mean the end of the treatment of patients with sebaceous carcinoma. Like other malignancies, patients must be the same as the long-term life-long follow-up. The degree of malignancy and recurrence rate of sebaceous carcinoma are serious. The growing attention and concern are catched by the domestic oncologists. Early detection and diagnosis and surgery are the key to improved survival which chemotherapy, radiation, frozen, and biological therapy are secondary. However, the current data is only limited in the short to medium term observation after effective treatment, there is no long-term follow-up reports of the results. With the continuous progress of modern treatment in the future, we must be able to find new means to remove the primary tumor cells and effective prevent their relapse. For Muir-Torre syndrome, the rare autosomal dominant disease, the clinical characteristics of patients are susceptible to sebaceous gland tumor (or multiple keratoacanthoma) and visceral malignancies. It have a common genetic basis With HNPCC. HNPCC gene mutation samely occur in the Muir-Torre syndrome, which hMSH2mutations are most common, followed by of hMLH1. When the clinical history and laboratory tests suggest Muir-Torre syndrome,it may be first to sequenc of the hMSH2gene. MSI detection of skin lesions is a easier method, it can be used as the basis of non-invasive screening of HNPCC or Muir-Torre syndrome and is a valuable method to determine the defects of the MMR genes and look for new MMR genes. Diagnosis and treatment of sebaceous carcinoma is also involved in other aspects of complex issues, such as the prospect of non-surgical treatment methods, new diagnostic and therapeutic methods development, molecular and genetic level research and so on, to be further explored.
