Objective:To investigate the relationship between plasma leptin, adiponectin and the degree of coronary heart disease(CHD), and to investigate the ideal doses of fluvastatin for treating CHD.Methods:The86patients with CHD confirmed by CAG were randomly divided into two groups(44cases in group1,42cases in group2). All of them were taken fluvastatin on the conventional therapy of CHD. Group1of conventional lipid-lowering group,40mg, daily; group2of intensive lipid-lowering group,80mg, daily. The plasma leptin and adiponectin were detected respectively by the enzyme-linked immunosorbent assay(ELISA) in these patients before treatment and one month after treatment. And the plasma leptin and adiponectin were detected in29non-CHD patients by the same way. Analysis the relationship between leptin, adiponectin with other biochemical indicators in different groups of patients.Results:1. In this study,86patients with CHD were selected and62patients were followed up, the followed-up rates was72%.31patients were followed up in both conventional lipid-lowering group and intensive lipid-lowering group, the follow-up rates were70%and72%.2. Compared with non-CHD, patients with CHD have higher level of plasma leptin and lower level of plasma adiponectin(P<0.05). The plasma levels of leptin was found to be positively correlated with the Gensini scores (r=0.949, P<0.05), whereas the level of plasma adiponectin was found to be negatively correlated with the Gensini scores (r为-0.892, P<0.05). The plasma levels of adiponectin was found to be negatively correlated with the level of plasma leptin(r=-0.949, P<0.05).3. Compared with before treatment, TC, LDL-c and plasma leptin were significantly decreased in both groups after one month(P<0.05), whereas adiponectin were significantly increased in these two groups(P<0.05).4. Compared with before treatment, there were no significant change in TG and HDL-c in both groups after one month(P>0.05).5. Compared with before treatment, there were no significant change in AST、 ALI、 GGT、and ALP in both groups, and there were no muscle pain patients in these two groups(P>0.05).6. Compared with conventional lipid-lowering group, the intensive lipid-lowering group has more lower level of TC, LDL-c, plasma leptin and more higher level of adiponectin (P<0.05), and intensive lipid-lowering group can significantly reduce plasma hs-CRP (P<0.05).Conclusion:1. There is a significant association between plamsa leptin and adiponectin with CHD. The plasma level of leptin was found to be positively correlated with the Gensini scores. With the degree of Gensini scores, the concentration of plasma leptin were gradually increased. The plasma level of adiponectin was found to be negatively correlated with Gensini scores. With the degree of the Gensini scores, the concentration of plasma adiponectin were gradually decreased. The plamsa level of leptin was found to be negatively correlated with the plamsa level of adiponectin, there was an complex relationship between them.2. Dyslipidemia is the most important risk factors of CHD. Fluvastatin can protect cardiovascular system by reducing the level of TC, LDL-c, and plasma leptin significantly, and elevating the level of plasma adiponectin in patients with CHD.3.The intensive lipid-lowering therapy can provide better protection than conventional lipid-lowering therapy by reducing the levels of TC, LDL-c, and hs-CRP significantly, while elevating the level of plasma adiponectin without adverse effects.
