objective:The aims of this study were to investigate the expression and clinical values of IGF-I and IGF-IR in colorectal cancer.Methods:1.60cases who underwent colorectal cancer operation.20cases who underwent colorectal adenoma operation and20non-tumor cases in the anal-intestinal department of surgery were randomly devided into three groups.For tumor patients, samples were obtained from the loci of cancer and loci of adenoma. And samples obtained from patients who underwent operation for non-neoplastic disease were used as control group.The expression of IGF-I and IGF-IR were studied by means of immunohistochemical assay(S-P method). The level of the expression was correlated with clinicopathological factors.2. The serum IGF-I level was determined by ELISA in pre-and postoperative cases of colorectal cancer.colorectal adenoma and the control group. Investigate the serum IGF-I level during three groups and compare sensitivity、specificity and accuracy during the detection of colorectal cancer with CEA and CA1-99.Results:1. The results of IGF-I and IGF-IR immunohistochemical expression in tissues:1.1The results of IGF-I immunohistochemical expression in tissues:IGF-I was mainly expressed on cancer cell cytoplasm. The positive expression rate of IGF-I in the loci of cancer.adenoma and control group was61.7%(37/60),30%(6/20),5.0%(1/20). The positive expression rate of IGF-I in the loci of cancer group was higher than that of adenoma and control group(P<0.05). The positive expression rate of IGF-I in the loci of adenoma group was higher than that of control group (P<0.05). The relationship between IGF-I expression and clinicopathological factors:There was a significant difference of the positive expression between the group with lymph node metastasis and the group without lymph node melastasis, between the group with serosa ivasion and the group without serosa invasion and between different Dukes grade(P<0.05). There was no significant difference correlation with age, sex.the tumor size and so on(P>0.05).1.2The results of IGF-IR immunohistochemical expression in tissues:IGF-IR was mainly expressed on cancer cell membrance. The positive expression rate of IGF-IR in the loci of cancer, adenoma and control group were70.0%(42/60)、45.0%(9/20)'10.0%(2/20). The positive expression rate of IGF-IR in the loci of cancer group was higher than that of adenoma and control group(P<0.05). The positive expression rate of IGF-IR in the loci of adenoma group was higher than that of control group (P<0.05). The relationship between IGF-IR expression and clinicopathological factors:There was a significant difference of the positive expression between the group with lymph node metastasis and the group without lymph node melastasis, between the group with serosa ivasion and the group without serosa invasion and between different Dukes grade(P<0.05). There was no significant difference correlation with age, sex,the tumor size and so on(P>0.05).2. There was no significant difference correlation with age, sex,the tumor size and so on(P>0.05).2.2The serum IGF-I level of the cancer group one week after operation was lower than that before operation (P<0.05).but higher than that of the control group(P<0.05). The serum IGF-I level of the cancer group one month after operation was lower than that before operation and one week after operation (P<0.05),but there was no significant difference between the cancer group one month after operation and the control group (P>0.05)2.3The sensitivity and specificity during the detection of colorectal cancer with IGF-I、CEA and CA1-99were higher than that with CEA and CA1-99,but there was no significant difference about accuracy between the different detection methods.Colusions:1. The positive expression rate of IGF-I in the loci of cancer group was higher than that of adenoma and the control group. This indicated that the higher IGF-Ⅰ and IGF-IR expression was correlated with the genesis of colorectal cancer.2. There was a significant difference of the positive expression between the group with lymph node metastasis and the group without lymph node melastasis, between the group with serosa ivasion and the group without serosa invasion. This indicated that IGF-I and IGF-IR were associated to colorectal cancer invasion and metastasis.3. The serum IGF-I level of the cancer group was higher than that of adenoma and the control group.There was a significant difference between the group with lymph node metastasis and the group without lymph node melastasis, between the group with serosa ivasion and the group without serosa invasion.4. There was no significant correlation between the positive expression rate of IGF-I and IGF-IR with age, sex,the tumor size and so on.5. The combined detection of IGF-Ⅰ、CEA andCA19-9can significantly improve the positive rate and accuracy of diagnosis in the colorectal cancer.6. IGF-I in the serum and in the tissue has a similar trendence, so IGF-I can be a feasible and promising tumor marker which serves as early diagnosis,progression and prognosis evaluation of the colorectal cancer.
