Comparison Of The Rotational Stability Among Different Types Of Intraocular Lenses:Effect On High-order Aberration

Objective:To compare the rotational stability in the capsular bag between three-piece and one-piece intraocular lenses(IOLs) and study the influence of rotation angle(RA) on the high-order aberrations(HOAs) of interior and entire eye with the anterior segment photography and iTrace; Furtherly investigate the rotational stability among different loop design of one-piece intraocular lenses,making full and comprehensive assessment of clinical effect on different types of IOL, in order to determine whether the design is suitable for toric lens.Methods:The prospective clinical study was divided into two parts. Firstly, a total of60eyes (60patients) were randomly divided into2groups. Group A implanted with AR40e three-piece hydrophobic acrylic foldable IOL.Group B implanted with ZCBOO one-piece hydrophobic acrylic foldable IOL. The uncorrected visual acuity (UCVA)and best corrected visual acuity(BCVA)were detected. The rotational stability was evaluated at the end of surgery using operating microscope photograph and then use digital anterior segment photograph among1day、1week and1month after surgery, and then use the Adobe Photoshop7.0software combined with MB-ruler to calculate the rotation angle. The high-order aberrations (HOAs) of interior and entire eye at one week, one month postoperatively under6mm pupil diameter were measured with iTrace visual function analyser and compared between the two groups.and then grouped with rotation angle comparing the HOAs between groups.Secondly, the other60eyes (60Patients) were randomly divided into2groups. Group C (30eyes), implanted with ZO one-piece hydrophilic acrylic foldable IOL; Group D implanted with QUATRIX one-piece hydrophilic acrylic foldable IOL combined with group B ZCBOO of the last research,make further evaluation of the rotational stability on different loop design of one-piece IOL.RA and vision (UCVA/BCVA) were measured and calculated like the last research.Results:There were no significantly differences on UCVA and BCVA postoperative among all eyes of four groups.There were no significantly differences between operative and one day postoperative in the four groups(P>0.05). The RA in Group A was(2.21±1.45)°nd(2.43±1.59)°,Group B was(1.98.±1.20)°and(2.14±1.37)°,Group C was(1.68±1.50)°and (1.78±1.55)°,Group D was(1.29±1.74)°and (1.34±1.81)°at one week and one month postoperative, there were no statistically significant differences between the two groups at the two junctures(P>0.05). The total high-order aberration(tHOA),spherical aberration(SA) of entire eye were statistically significantly lower in group B than in group A at one week and one month postoperatively(P<0.05),and the SA of interior eye was also significantly lower(P<0.05).There were no statistically differences of HOAs between RA<3°group and RA≥3°group in the two groups at one week and one month postoperatively (P>0.05).There were also no statistically significant differences among Group B、C and D(P>0.05) at one week and one month postoperative.Conclusion:They all demonstrate good rotational stabilities in three-piece and one-piece IOL with different loop design, especially in one-piece IOL with multiple loops, which is suitable for toric IOL.The rotation angles of three-piece and one-piece IOLs have no influence on HOAs.
