| Objective:To compare visual quality including high-order aberration, modulation transfer function (MTF) and depth of focus (DOF) of the eyes implanted with two different types of aspheric intraocular lens (IOL) and spheric IOL at different pupil diameter. It is used to provide some reference for reasonable correction of spherical aberration.To analyze high-order aberration, MTF(tHOA) and the changes of MTF value with or without different Zernike mode in the eyes implanted with two different aspheric IOLs, and then analyze the main factor of high-order aberration in pseudophakic eyes affected MTF(tHOA).It is used to guide targeted correction of high-order aberration.Methods:1. A total of80patients (90eyes) were divided into three groups randomly. Group I implanted with Tecnis ZA9003aspheric IOL, Group II implanted with Akreos Adapt AO aspheric IOL, Group III implanted with Acrysof SA60AT spheric IOL.The iTrace was used to measure corneal spherical aberration C40, total ocular high-order aberration, total ocular MTF(tHOA) at3mm and5mm pupil diameter. The Visual Optics Lab-CT software was used to measure the DOF at5mm pupil diameter.2. A total of90patients (100eyes) were divided into two groups randomly. Group I implanted with Tecnis ZA9003IOL, Group II implanted with Akreos Adapt AO IOL. The iTrace was used to measure corneal spherical aberration C40, total ocular high-order aberration, total ocular MTF(tHOA), MTF value with or without different Zernike mode (C3-3,C3-1,C31,C33,C40) respectively at6mm pupil diameter. The independent samples t test and paired-t test was used to assess the date.Results:1. When at3mm pupil diameter, the value of corneal spherical aberration C4have no difference between three groups (P>0.05); total ocular spherical aberration C40was lower in Group I than in Group III,while total ocular third order coma, third order trefoil, total high-order aberration and MTF(tHOA) value have no difference between three groups (P>0.05). When at5mm pupil diameter, the value of corneal spherical aberration C40have no difference between three groups (P>0.05); total ocular spherical aberration, high-order aberration, MTF(tHOA) value and DOF were different between the three groups (P<0.05), while third order total ocular coma, third order trefoil have no difference (P>0.05).2. When at6mm pupil diameter, the value of corneal spherical aberration C40have no difference between three groups (P>0.05); total ocular spherical aberration C40and total high-order aberration were lower in Group I than in Group II (P<0.05).The total ocular MTF value without C40was not significantly different between the two groups (P>0.05), while MTF(tHoA) and the MTF value without C3"3,C3-1,C31,C33respectively were significantly different between the two groups (P<0.05). When in group I, the MTF value improved at5-30cpd without C3-3,C3-1,C3I,C33respectively and there was no significantly improvement of MTF value without C40at5-30cpd, The MTF value got the most improvement without C3-3.When in group â…¡,the MTF value improved at5-30cpd without C3-3,C3-1,C31,C33,C40respectively and the MTF value got the most improvement without C40.Conclusion:When at3mm pupil diameter, the eyes implanted with aspheric IOL with negative spherical aberration have a lower value of spherical aberration C40than the eyes implanted with spheric IOL, total high-order aberration and MTF(tHOA) value for total ocular have no difference between the eyes implanted with aspheric IOL and spheric IOL. When at5mm pupil diameter, the eyes implanted with aspheric IOL with negative spherical aberration have a high value of MTF(tHoA), but sacrificing tolerance to defocus; the eyes implanted with aspheric IOL with zero spherical aberration have a high value of MTF(tHOA) compare with spherical IOL at5-20cpd, without sacrificing tolerance to defocus; while eyes implanted with spheric IOL have a high value of DOF,but MTF(tHOA) value was lower. Visual quality of the eyes implanted with different types of IOL need comprehensive analysis, it is better to reserve some positive spherical aberration to raise the visual quality. MTF(tHOA) value was different between the eyes implanted with the IOL with negative spherical aberration and zero spherical aberration at6mm pupil diameter and the spherical aberration C40was responsible for the difference,vertical trefoil C3"3show a more significant effect on the eyes implanted with Tecnis ZA9003IOL, spherical aberration C40show a more significant effect on the eyes implanted with Akreos AO IOL. |
