The Effect Of Dietary Continuous Nursing Intervention On Type2Diabetes Patients

[objectives]To set up the dietary continuous nursing intervention to the type2diabetes patients, and to compare the effectiveness of a dietary continuous nursing program versus a routin care for type2diabtes.[Methods]The research designed in this study was a randomized controlled trial.86type2diabetes patients were recruited and randomly assigned into experimental group (46participants) and (45participants). The patients in the control group received the routine guidance, and the patients in the experimental group received the routin care and a dietary nursing intervention, including targeted intervention plan, individual education, educational materials and instrument, dietary behavior intervention, and6months follow-up visit. Biochemical indicator, human body measurement index were collected at baseline,3and months after the intervention. Quality of life and depressive symptoms were assessed using diabetic quality of life (DQOL) questionnaire and the Zung Self-Rating Depression Scale (ZSDS). And the dietary knowledge, attitude and dietary behaviors were assessed by a diabetes dietary knowledge, attitude and behaviors questionnaire which was developed by the researchers at baseline,3and6months after the intervention. Chi-square test, independent-samples t test, repeated measure ANOVA, one-way repeated measures ANOVA and Mann-Whitney U test were conducted to the data.[Result]In the experimental group, the fasting blood glucose, blood sugar2hours after meal glycolated hemoglobin, daily diet glycemic load, and the score of the treatment aspect of diabetic quality of life (DQOL) questionnaire were lower at3and6months than the pre-intervention, the score of dietary knowledge, attitude and behaviors questionnaire was higher at3and6months than the pre-intervention(P<0.01). Total cholesterol and systolic pressure at6months post-interventions were lower than before (P<0.05). Triglyceride, diastolic pressure, body mass index, waist to hip ratio, score of quality of life (DQOL) questionnaire, the scores of the psychology and social relation of the quality of life questionnaire, and and score of Zung Self-Rating Depression Scale was declined, but there is no statistical significance(P>0.05). Between the two group, the fasting blood glucose, blood sugar2hours after meal glycolated hemoglobin, daily diet glycemic load, body mass index, and score of the treatment aspect of diabetic quality of life (DQOL) questionnaire of the experimental group, were lower (P<0.05) and the score of dietary knowledge, attitude and behaviors questionnaire was higher than the control group at the3and6month (P<0.01). Total cholesterol, systolic pressure were also lower than the control group at the3month (P<0.05).[conclusion]Dietary control is an important part of the type2diabetes patient. Dietary continuous nursing intervention model can reduced the the fasting blood glucose, blood sugar2hours after meal glycolated hemoglobin, daily diet glycemic load, Total Cholesterol systolic pressure,daily diet glycemic load, and increase the dietary knowledge of the patients, and improve their attitude and promote the correct dietary behavior, and therefore endance their dietary compliance.
