A Comparative Study Of B-ultrasound And DSA In The Diagnosis Of Carotid Stenosis

Objective Analysis the conformity of B-ultrasound with DSA，whichis golden standard，in the diagnosis of carotid stenosis.Discuss thecharacteristics and clinical value of B-ultrasound.Methods79consecutive hospitalized patients in the Department ofNeurology from September2010to March2011were included. All of thesubjects were diagnosed TIA，cerebral infarction or posterior circulationischemia according to the4thnational cerebrovascular conference diagnosiscriteria. The evaluation of carotid stenosis was respectively conducted byB-ultrasound and DSA examination within one week successively. Comparingwith DSA, parameters of B-ultrasound such as sensitivity（Sen），specificity（Spe），positive predictive value（PRV）and negative predictive value（NPV）were calculated and analysised.Results In79patients, from158carotid arteries examined by ultrasound and DSA,118blood vessels showed the same condition, coincidence rate was74.7%,ultrasound diagnosis was slightly higher in16blood vessels, with falsepositive rate for10.1%,the diagnosis slightly decreased in24blood vessels, withfalse negative rate for15.2%.The sensitivity, specifity, accuracy, positivepredictive value of ultrasound in diagnosis of mild carotid stenosis were92.6%，69.6，78.1%;In Moderate stenosis, they were66.7%，94.1%，57.1%;In severedegree stenosis, they were54.5%，96.9%，75%;In obstruction, they were100%、96.2%、96.6%、75.0%、100%. The total Kappa value was0.655.Conclution The accuracy of ultrasound evaluation for carotid stenosis ishigh and the results have moderate relevancy with DSA.
