| Objective:To explore the treatment method of type C pelvic fracture through retrospective study on the effective of the fracture,including the fracture reduction,short-term function and long-term function after operatio,treated by posterior percutaneous plate combining with anterior external fixationn.Methods:The clinical information(including medical record,imaging material and follow-up data) of the patients who were suffered from the pelvic ring fracture treated by posterior percutaneous plate combining with anterior external fixation form May2008to May2011in Orthopedics of Wuhan General Hospital of Guangzhou Command was be collected and analyzed through the electronic medical records system and PACS system. The research objects were selected based on inclusion criteria,exclusion criteria.According to the following-up plan draw up in the study, the contents such as the symptoms, signs and imaging data were regularly collected. The biggest fracture displacement of sacroiliac joint was obtained from the postoperative pelvic X-ray and CT images according to Matta standards. The function of the hip was evaluated by Majeed standards.All the data have been gotten was analyzed by SPSS13.0software.Results:There were382cases with pelvic ring fracture admitted to the department form May2008to May2011. According to the inclusion and exclusion criteria,20cases were selected,but the actual cases in the study were18for2cases of them fall off. All of the cases were treated by posterior percutaneous plate combining with anterior external fixation. According to Tile classification system, there were3type C1,5type C2,10type C3. All patients were followed-up for6to30months(mean12months). The average operative time of simple pelvic fracture surgery was38minutes, the average amounts of bleeding was35ml, there was not incision infection happened in this group of patients. To evaluated the reduction of fracture according to Matta standards, the results as fellow:excellent in3, good in10, fair in1, poor in0, with excellence rate of72.2%. To evaluated the function according to the Majeed standards, the results as fellow:excellent in4, good in10, fair in3, poor in1, with excellence rate of77.8%. The marks of the reposition and function was statistically significant.Conclusion:The combinative fixation is a effective treatment to the pelvic ring fracture of type C for its minimal invasion, simple procedure and reliable function. Is an effective method for the treatment of Tile C pelvic fractures. But its anti-longitudinal stress is weaker, meanwhile it is lack of bulkã€multi-centerã€long-term efficacy of observation and the biomechanical research, so it needs to be further in-depth observed and researched. |
