| Objectives To compare the clinical effects among screw rod external fixation and plate internal fixation of 1/3 displaced midshaft clavicle fractures.Methods Retrospective analysis was conducted on 207 patients with clavicle fractures in the peoples' Hospital of jiangxi province from January 2008 to September2015,and 54 of these patients met the inclusive criteria.Among them,there were 28 eases of external fixation(Group A),26 cases of plate fixation(Group B).The results were evaluated from operative time,blood loss,healing time,the VAS,the Neer score of the shoulder function after operation 12 months and DASH.Results All patients were followed up 12 to 24 months,the patients of screw rod external fixation groups were less than plate internal fixation groups in operative time and blood loss,P<0.05,had difference statistically significance.The healing time of screw rod external fixation groups was shorter than plate internal fixation groups,P<0.05,had difference statistically significance.The VAS after operation 1day and 2days and 3days of in the patients of plate internal fixation groups was higher than screw rod external fixation groups,P<0.05,had difference statistically significance,but the VAS after operation 7 days and 1 year had not difference statistically significance.Based on the comparison of the Neer score of the shoulder function after operation 12 month among the two groups,there was no significant difference between screw rod external fixation groups and plate internal fixation groups.Mean values of DASH of plate internal fixation groups were lower than screw rod external fixation groups.Conclusion In the treating 1 / 3 displaced midshaft clavicle fractures in adults,screw rod external fixation are overall better than plate internal fixation,Both external fixation and plate fixation have its advantages and disadvantages.The choice of treatment must be based on the clinical type of the fracture and the communication with patients. |
