A Study On The Effect Of Individual Health Education On Improving The Sleeping And Anxiety And Depression Of Tumor Patients With Treatment Of Microwave Ablation Surgery

Objective:To evaluate the effect of whole course individual health education mode (The new hospital patients guidance, preoperative and postoperative guidance, and discharge guidance guide according to the patients’ conditions and demands for such patients for their own customized health education plans) on improving tumor patients’ sleeping quality and anxiety and depression with treatment of microwave ablation surgery thus providing the scientific basis and effective health education plans formulated to patients with this tumor treatment for nursing workers.Methods:A sample of102tumor patients were recruited from the Department of Gastroenterology, A third-grade class-A hospital of shandong provincial during July2010to November2011. They were evenly assigned into the control group (51cases) and the experimental group (51cases) according to the patients’ admission time. The conventional health education was provided to the control group, while the experimental group were brought into the whole course individual health education mode. Measurements of sleeping quality, anxiety and depression were respectively taken by Pittsburgh Sleep Quality Index Scale (PSQI), State Anxiety Inventory, Center for Epidemiological Studies depression Scale at three time points (the second day after admission, the morning before operation, the day of their discharge). Besides, the general information was taken by the general condition questionnaire.Results: 1. The mean score on Pittsburgh Sleep Quality Index of all the patients is8.90+2.11on the the second day after admission, which is higher than Domestic norm (7).No statistical significance was obtained between the control group and the experimental group (p>0.05). After intervention, there was a sharper trend of significantly declined scores on Pittsburgh Sleep Quality Index in the experimental group than in the control group (p<0.05). Pittsburgh Sleep Quality Index score in the experimental group was significantly lower than that of the control group on the morning before operation and the day of their discharge (p<0.05).2. There was no statistical significance in subjective sleep quality (composition Ⅰ), sleep time (composition Ⅲ), sleep effect (composition Ⅳ), sleeping medication use(composition Ⅵ), daytime function (composition Ⅶ) dimension between the control group and the experimental group on the admission day (p>0.05). After intervention, there was a sharper trend of significantly declined scores on each dimension in the experimental group than in the control group (p<0.05). Scores of the above dimensions in the experimental group was significantly lower than the control group on both of the morning before their operations and the day of their discharges (p<0.05). The scores of the sleeping time, sleep disturbances dimensions in the PSQI at each time point and their change trends show no statistical difference between the two groups (p>0.05).3. The mean anxiety score of all the patients (61.57±7.21) was significantly higher than that of national norm (39.91) on admission day (p<0.05). Meanwhile, the mean depression score of all the patients(20.42±5.31) was significantly higher than that of national norm (11.52) on admission day (p<0.05). There is no statistical significance in anxiety score and depression score between the experimental group and the control group (p>0.05). After intervention, there was a sharper trend of significantly declined scores on anxiety dimension and depression dimension in the experimental group than in the control group (p<0.05). Anxiety score and depression score in the experimental group were both significantly lower than that of the control group on the morning before operation and the day of their discharge (p<0.05). Conclusions:The whole course individual health education mode can provide abundant content and diverse forms, thus it can to a large extent meet patients’needs of information and emotional sustenance. Meanwhile it is an effective approach in reducing uncertainty in illness, anxiety and depression.
