Objective:To investigate the expression of Osteopontin(OPN) and Vascular endothelial growth factor(VEGF) in primary hepatocellular carcinoma, and their correlation of expression. Discusse the relationship between OPN、VEGF and clinicopathologic features. Analysis the role of OPN and VEGF in cancer invasiveness.Methods:Using the immunohistochemical staining to examine the expression of OPN and VEGF in48cases of primary hepatocellular carcinoma, and in11cases of adjacent hepatic tissues to be as controls. Analysis the relationship of OPN、 VEGF with clinicopathologic features, such as gender、age、serum concentration of AFP、liver cirrhosis、HBsAg、the Child-Pugh classification、tumor number、tumor size、TNM stage、tumor differentiation、tumor thrombus、intrahepatic or extrahepatic metastasis、tumor capsule、liver capsule invasion and so on.Results:1. The positive expression of OPN is located in cytoplasm of cells, showing yellow or brown, and uniformly distributed. In48cases of HCC,34cases of OPN expression is positive, the OPN-positive expression rate is70.83%. In11cases of adjacent hepatic tissues, only two cases are expressed as positive, the positive expression rate was18.18%. between the HCC tissues and adjacent hepatic tissues there are significant differences(χ2=10.43, P=0.001). In intrahepatic or extrahepatic metastasis of HCC cases, the OPN-positive expression rate(92.31%,12/13) is higher than the OPN-positive expression rate(62.86%,22/35) those without metastasis, P=0.046. HCC with tumor thrombus, the OPN-positive expression rate(100%,8/8) is higher than the OPN-positive expression rate(65%,26/40) those without tumor thrombus, P=0.047. HCC with incomplete capsule or without capsule, the OPN-positive expression rate (80.65%,25/31) is higher than the positive expression rate of encapsulated HCC tissue (52.94%,9/17), P=0.043. invasive liver capsule by HCC tissues, the OPN-positive expression rate (85.71%,18/21) is higher than the positive expression rate(59.26%,16/27) those liver capsules are not invasion, P=0.045. tumor diameter≥5cm, the OPN-positive expression rate (82.14%,23/28) is higher than the positive expression rate (55%,11/20)with tumor diameter <5cm, P=0.041. TNM stage Ⅲ、Ⅳ, the OPN-positive expression rate (86.36%,19/22) is higher than in stage Ⅰ、Ⅱ cases (57.69%,15/26), P=0.029. The OPN-positive expression rate is not significantly correlated with gender、age、serum concentration of AFP、liver cirrhosis、HBsAg、the Child-Pugh classification、tumor number、tumor differentiation, P>0.05.2. The positive expression of VEGF is located in cytoplasm of cells, showing yellow or brown, and uniformly distributed. In48cases of HCC,32cases of VEGF expression is positive, the VEGF-positive expression rate is66.67%. In11cases of adjacent hepatic tissues, two cases are expressed as positive, the positive expression rate was18.18%. between the HCC tissues and adjacent hepatic tissues there are significant differences(χ2=8.615, P=0.003). In intrahepatic or extrahepatic metastasis of HCC cases, the VEGF-positive expression rate(92.31%,12/13) is higher than the VEGF-positive expression rate(57.14%,20/35) those without metastasis, P=0.022. HCC with tumor thrombus, the VEGF-positive expression rate(100%,8/8) is higher than the VEGF-positive expression rate(60%,24/40) those without tumor thrombus, P=0.028. HCC with incomplete capsule or without capsule, the VEGF-positive expression rate (77.42%,24/31) is higher than the positive expression rate of encapsulated HCC tissue (47.06%,8/17), P=0.033. invasive liver capsule by HCC tissues, the VEGF-positive expression rate (85.71%,18/21) is higher than the positive expression rate(51.85%,14/27) those liver capsules are not invasion, P=0.014. tumor diameter≥5cm, the VEGF-positive expression rate (78.57%,22/28) is higher than the positive expression rate (50%,10/20)with tumor diameter<5cm, P=0.038. TNM stage Ⅲ、Ⅳ, the VEGF-positive expression rate (86.36%,19/22) is higher than in stage Ⅰ、Ⅱ cases (50%,13/26), P=0.008. The VEGF-positive expression rate is not significantly correlated with gender、age、serum concentration of AFP、liver cirrhosis、HBsAg、the Child-Pugh classification、tumor number、tumor differentiation, P>0.05.3. in the48HCC cases, both OPN and VEGF with positive expression are30cases, both OPN and VEGF with negative expression are12cases, the OPN positive and VEGF negative expression are4cases, the OPN negative and VEGF positive expression are2cases. Spearman correlation, between OPN and VEGF expression is a positive correlation (r=0.673, P<0.001).Conclusions:1.OPN and VEGF are slignificantly correlated with intrahepatic or extrahepatic metastasis、tumor thrombus、tumor capsule、liver capsule invasion、tumor size、 tumor differentiation、TNM stage. These clinical features are closely related to HCC invasive, so the OPN and VEGF play a key role in invasion and metastasis of hepatocellular carcinoma. There is positive correlation and promote between expressio of OPN and VEGF in hepatocellular carcinoma.2. The combinatory investigation of OPN and VEGF may be helpful to judge the prognostic of patients with hepatocellular carcinoma.3.The expression characteristics of OPN and VEGF in hepatocellular carcinoma will to provide important targets for molecular targeted therapy of hepatocellular carcinoma.
