Objectives:Investigate the value of the Thinprep Cytologic Test, colposcopy, colposcopic directed biopsy, loop electro surgical excision procedure (LEEP) in the diagnoses of cervical disease.Methods:A series of905cases who had cervicitis and cerical intraepithelial neoplasia was diagnosed by cervical colposcopic directed biopsy in QILU Hospital of ShanDong University from June2010to January2012. All of them have done the TCT before the colposcopic directed biopsy.119cases whose were cervical intraepithelial neoplasia(CIN I or above) were done LEEP and analysis the pathological results of before operation and after. To compare the patient’s ages, the results of TCT and colposcopic directed biopsy, colposcopic and colposcopic directed biopsy, colposcopic directed biopsy and the pathological findings of LEEP. And investigate the value and significance of all the methods above in the diagnosis of cervical disease.Results:1. There are905cases patients who were diagnosed with TCT, and the results of748cases patients were abnormal. Of the748patients, there are50cases who were found a small amount of typical cells and the proportion is6.68%(50/748); ASCUS38.77%(290/748);AGC1.74%(13/748);ASC-H9.63%(72/748);LSIL21.26%(159/748);HSIL20.32%(152/748);SCC1.60%(12/748);2. All the905 cases patients were diagnosed with TCT and colposcopic directed biopsy. The coincidence rate between TCT and colposcopic directed biopsy are as follow:40.00%(20/50) in the patients whose results were found a small amount of typical cells; ASCUS40.34%(117/290),AGC53.85%(7/13),LSIL64.15%(102/159) ASC-H74.03%(53/72), HSIL82.89%(126/152), cervical cancer91.67%(11/12), the total coincidence rate58.23%(527/905);3. The coincidence rates between colposcopy and colposcopic directed biopsy are as follow:inflammation73.70%(297/403), CIN Ⅰ60.00%(153/255), CIN Ⅱ60.99%(86/141), CINⅢ89.29%(75/84), cervical cancer86.36%(19/22), the total coincidence rate69.61%(630/905);4.119cases patients whose biopsy results were CIN I and above were operated with LEEP. Comparing the biopsy results before operation with the results after operation and their coincidence rates are as follow:the total coincidence rate66.39%(79/119), CIN Ⅰ60.71%(17/28), CIN Ⅱ63.27%(31/49),CINⅢ3.81%(31/42)Conclutions:1. The CIN occurs easily in the female between20years old to40;2. Now the TCT is still the principal screening method in clinical, expecially in the cervical carcinoma and high-grade cervical intraepithelial neoplasia;3. The colposcopic directed biopsy can not take the place of the LEEP in the diagnose of the cervical disease;4. LEEP is an effective method in the diagnosis and treatment of the cervical intraepithelial neoplasia and it is worth to widely practiced.
