OBJECTIVES:Generally, community hospitals are such medical organizationsinferior to the first-level hospitals, most of which are small in scale andwidely distributed with variety of served population. However, many ofthem have disadvantages, such as obsolete facilities, irrational management,and insufficient resources. Therefore, it was much important to doretrospective study on the medical records of inpatients in order to masterthe epidemiology of diseases, hold the effectiveness and rationality of druguses, standardize clinical drug applications, and improve service quality.METHODS:Epidemiological survey methods were used to collect10039copies ofinpatients’ medical records with sex, age, AD, DD, drug uses,hospitalization days, and therapeutic outcome, from internal medicine andoncology departments during the period of May2010to June2011in acommunity hospital of Chongqing.Excel table was used to do statistic work of the data above and obtain relative information of age distribution, average age, maximal age, numberof patients with more than≥60, sex ratio, average hospitalization days,number of patients with less1day-hospitalization, identical diagnosis ratioof AD to DD, distribution of disease system, and number of medicationused in patients, which were expressed in tables or figures.We put stress on the survey of antibiotics and Chinese patent druginjections. The contents of former included use frequency of antibiotics,ratio of preventive medication to curative medication, combination uses ofantibiotics; the later included use conditions of Chinese patent drugs,combination uses of western drugs and traditional drugs, and probable ADRof Chinese patent drug. The rationality of drug uses was analyzed and somesuggestions were presented.RESULTS:1. The average age of inpatients was60.5±11.1with55.54%of overthan60years old; average hospitalization time was9.7d with9.94%of1d-hospitalization. The diseases inpatients suffered from mainly chroniccardio-cerebral vascular diseases, chronic respiratory diseases, endocrinediseases (diabetes mellitus), and gastro-intestinal diseases.2.5026(50.16%)in10039inpatients had taken antibiotics amongwhich number of patients using antibiotics for therapeutic or preventivepurposes was3584(71.17%) and1452(28.83%),respectively. Thelater mainly came from neurological and oncological departments.2622 patients(52.06%)took more than two sorts of antibiotics in combinationuse,including1695cases using two sorts of antibiotic drugs at same time.The top ten antibiotics widely used in this hospital were as follows:levofloxaxin, gentamicin, mezlocillin sodium, cefmetazole, zinacef,sulperazone, cefoxitin, ceftazole sodium, penicillin sodium, and mezlocillinsodium/sulbactam sodium.3.2375(23.66%) in10039inpatients only took western medicinewhile number of inpatients only took chinese patent medicine;the was53(0.53%), the rest inpatients (7325,72.97%) took both at the same time.The top four Chinese patent medicine used in this hospital was as follows:acutase, Honghua injection, Shuxuetong injection, Tanreqing injection.CONCLUSIONS:1. The inpatients of this hospital mainly are old people with variety ofchronic diseases and metabolic diseases. The frequency of drug usesdepends on the patients`situation and diseases. Drugs widely used areanticoaguants, thrombolytics, antihypertesives, vasodilators, insulinpreparations, and proton pump inhibitors, except antibiotics.2. There exists a high use frequency of combined Western-Chinesemedicine in this hospital, but it is needed for medical workers to strengthentraining for indication selection, therapeutic observation, and recognition ofpossible adverse reactions.3. β-lactone antibiotics have the highest use frequency among the all antibiotics. The antibiotic applications in this hospital are well-controlled,but it is necessary to further improve the skills of combination therapy andpreventive medication of antibiotics.4. There is a wide insufficiency of ADR recognition with untidyrecords, even loss in this hospital. This reminds the hospital to strengthenthe management of ADR records.
