Objective:To explore the relationship of IL-18and other cell factors serum levels,andrelevant clinical relationship between genotype B and genotype C in the chronichepatitis B(CHB)patients.Methods:103patients with CHB were included with genotyping detection of B, C bygenotyping fluorescent assay of the hepatitis B virus (HBV). The serum levels ofIL-18、IL-4、IL-12、IFN-γ in patients with CHB and healthy persons were detected bydouble-antibody sandwiched ELISA. The differences between IL-18and other cellfactors expressed in patients with CHB and normal controls and in differentgenotypes were compared.Results:①Of103patients with CHB,75cases (72%) were genotype B,17cases (17%)were genotype C,4cases (4%) were BC mixed genotype,7cases (7%) wereuntypied.②The proportion of genotype B was higher than genotype C in mild chronichepatitis B、moderate chronic hepatitis B、severe chronic hepatitis B、heavy chronichepatitis B and liver cirrhosis groups. Except for the proportion of genotype C inheavy Chronic hepatitis B was higher than it in mild chronic hepatitis(P<0.05), thedistribution of genotype B and genotype C didn’t have significant difference in othergroups(P>0.05).③T he proportionof HBeAg-positive cases in genotype C group was higher thanin it genotype B group, but with no significant difference(P>0.05). The proportionof HBeAg-positive cases was no significant difference between genotype B groupand genotype C group. The serum levels of HBVDNA in genotype C group wassignificantly lower than it in genotype B group(P<0.05).④T hedistribution of genotype C in moderate viral load group was higher than it in high viral load group of all HBVDNA positive cases with CHB (P<0.05.And theIL-18level in low and moderate viral load group were obviously higher than it inhigh viral load group(P<0.05).⑤T hesecretion levels of IL-18、IL-12、IFN-γ in cases of genotype C wassignificantly increased than in cases of genotype B with CHB(P<0.05), but there wasno significant difference in the secretion levels of IL-4(P=0.057).⑥T helevels of IL-18、IL-12、IFN-γ in every group with CHB were higher thanin normal control group. The secretion level of IL-18in other groups except in severechronic hepatitis B and liver cirrhosis groups, was in rising trend with liver diseaseaggravated. Compared in normal control group, the level of IL-4was increased inmild chronic hepatitis B and moderate chronic hepatitis B groups and was decreasedin heavy chronic hepatitis B group.⑦T he levels of IL-18、IL-12、IFN-γ were positive correlated while the level ofIL-4was negative correlated with ALT、AST、TBIL of cases with CHB.Conclusion:103cases of chronic hepatitis B patients mainly B genetype, as C genotype mayprefer distributed in serious liver disease patients.Meanwhile, genotype C might bemore inclined to be distributed in more serious liver disease. The secretion levels ofIL-18、IL-12、IFN-γ was related with the severity of disease and liver inflammationactivities, the IL-4secretion was restrained when liver inflamm-ation activitiesexacerbated. The immune level of Th1cell was in patients genotype C with strongerthan in patients with genotype B when liver inflammation of CHB was proceeding,while the degree of suppression of Th2cell immune might be higher.
